- Abdominal Aortic Aneurysm (AAA): The most deadly mimic. In older patients presenting with acute, severe, nontraumatic flank pain, a ruptured or expanding AAA must be considered before a kidney stone.
- Infected Obstructing Stone (Urosepsis): Concomitant obstruction and infection rapidly destroy the renal parenchyma and cause systemic sepsis.
- Ectopic Pregnancy / Ovarian Torsion: Must be ruled out in reproductive-age females presenting with flank or lower quadrant pain.
- Prioritized Diagnostic Workup:
- Basic Labs: Order a CBC, BUN/Creatinine (to assess baseline renal function and rule out acute renal injury), and a Urinalysis (UA) with Urine Culture.
- Imaging: Point-of-Care Ultrasound (POCUS) is the preferred initial modality to evaluate for hydronephrosis. If further anatomical definition or surgical planning is required, a non-contrast CT is the gold standard.

- Non-Contrast CT: Look for the hyperdense calcification within the ureteral tract. Measure the stone precisely: stones <3 mm are highly likely to pass spontaneously, whereas stones >4 mm have a lower likelihood, and a proximal stone >6 mm indicates high-grade obstruction requiring intervention.

- Cognitive Trap (The "Simple" Stone): Discharging a patient with an obstructing stone who also has a positive urinalysis. Correction: An infected stone behind an obstruction is a closed-space infection that rapidly leads to irreversible renal damage. This is a critical urologic emergency requiring prompt IV antibiotics and admission.
- Critical Action: Always review the patient's baseline renal anatomy. A stone in a patient with a solitary kidney acts as a bilateral obstruction, mandating immediate urologic decompression.
