A Urinary Tract Infection (UTI) is an acute inflammatory response of the urothelium to invading microorganisms, most commonly Escherichia coli, which typically ascend from the skin or rectum. If the infection ascends beyond the bladder into the upper urinary tract (acute pyelonephritis), it can cause severe systemic toxicity; consequently, the genitourinary tract is the most common source of infection in patients presenting with septic shock, carrying a mortality rate of 10% to 20%. In poorly controlled diabetics, specific gas-forming organisms can cause emphysematous pyelonephritis, a severe, life-threatening necrotizing infection of the renal parenchyma.

Hematuria is defined as the presence of red blood cells in the urine and occurs when the vascular-urothelial barrier is breached. It is mechanistically divided into glomerular causes (e.g., glomerulonephritis, classically presenting with RBC casts) and non-glomerular causes (e.g., nephrolithiasis, BPH, malignancies, or infections).

2. THE BEDSIDE ACTION PLAN (Rapid ER Management)

  • Immediate Stabilization: Assess ABCs, establish IV access, and evaluate for signs of septic shock (hypotension, tachycardia, altered mental status). If signs of sepsis are present, aggressively resuscitate with IV isotonic crystalloids and administer broad-spectrum parenteral antibiotics (e.g., Ceftriaxone 1 g IV) early.
  • Uncomplicated Cystitis (Females): The first-line oral regimens are Nitrofurantoin (5 days), a single dose of Fosfomycin, or Trimethoprim-Sulfamethoxazole (TMP-SMX) 160/800 mg (3 days, provided local resistance is <20%). Fluoroquinolones are explicitly not recommended as first-line therapy for uncomplicated UTIs.
  • Complicated UTI (Males): UTIs in men are considered complicated. Treat with a 7-day course of antibiotics (e.g., Nitrofurantoin 50 mg qds or Trimethoprim 200 mg bd) and actively evaluate for alternative diagnoses like prostatitis or epididymitis.
  • Uncomplicated Pyelonephritis (Outpatient): For healthy females tolerating oral intake, discharge is safe. You may consider administering a single dose of IV Ceftriaxone prior to discharge if prescribing an oral fluoroquinolone or TMP-SMX to hasten symptom resolution; however, an IV dose is not recommended if discharging on an oral cephalosporin.

3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

  • Obstructing Infected Calculus: A true urologic emergency requiring immediate operative drainage.
  • Emphysematous Pyelonephritis: Suspect in poorly controlled diabetics with sepsis.
  • Urologic Malignancy: Must be heavily suspected in older patients presenting with painless gross hematuria.
  • Penetrating or Blunt GU Trauma: Hematuria is an unreliable marker for penetrating trauma, mandating advanced imaging based on wound proximity.
  • Prioritized Diagnostic Workup:
  • Urinalysis (UA): Used for rapid ED diagnosis. Sensitivity is enhanced if any of leukocyte esterase, nitrites, protein, or blood are positive. A negative dipstick in a patient with a high pretest probability should prompt a search for alternative diagnoses.
  • Urine Microscopy: Crucial for differentiation. The presence of WBC casts indicates pyelonephritis, whereas RBC casts indicate glomerulonephritis.
  • Urine Culture: Send cultures for all upper UTIs (pyelonephritis), males, children, pregnant patients, or when antibiotic resistance is suspected.

4. THE VISUAL BOARD (ECG / POCUS / Imaging)

  • Point-of-Care Ultrasound (POCUS): Perform a bedside renal ultrasound on any patient with a UTI and signs of sepsis to rapidly evaluate for hydronephrosis (indicating an obstructing stone). In diabetic patients with severe flank pain, explicitly look for hyperechoic areas within the renal sinus with "dirty" posterior acoustic shadowing, which is the pathognomonic sign of gas in emphysematous pyelonephritis.
  • CT Abdomen/Pelvis (CT Urogram): The gold standard imaging modality. Use non-contrast CT to identify simple nephrolithiasis. Use an IV contrast-enhanced CT with delayed excretory images (10-minute delays) to evaluate for urologic malignancy in cases of painless hematuria, or to assess for ureteral contrast extravasation in patients with penetrating flank trauma.

5. THE SCORING MATRIX (Risk Stratification & Guidelines)

  • Clinical Decision Unit (CDU) / Admission Exclusion Criteria: Patients with pyelonephritis are excluded from short-term CDU observation and mandate formal hospital admission if they exhibit any of the following: pregnancy, hemodynamic instability (SBP <90, HR >120), significant comorbidities (diabetes, renal failure, sickle cell disease), immunosuppression (HIV, transplant), or an anatomic urologic abnormality (stones, stents, strictures).
  • Blunt GU Trauma Imaging Indications: Emergent CT imaging of the urinary tract is strictly indicated only if the patient has: (1) Gross hematuria, (2) Microscopic hematuria combined with shock (SBP < 90 mm Hg), or (3) A rapid deceleration mechanism highly concerning for renal pedicle injury. Stable patients with isolated microscopic hematuria do not require emergent CT and can be discharged with urology follow-up.

6. THE DANGER ZONE (Pitfalls & Critical Actions)

  • Cognitive Trap (Treating the Urinalysis): Treating asymptomatic bacteriuria or asymptomatic pyuria with antibiotics. Correction: Bacteriuria without clinical symptoms should not be treated unless the patient is pregnant or severely immunosuppressed. In ESRD patients on hemodialysis, asymptomatic pyuria is extremely common due to urinary stasis and does not indicate infection without systemic signs.
  • Cognitive Trap (The Anticoagulant Excuse): Dismissing painless gross hematuria in a patient taking warfarin or DOACs as simply a "side effect of blood thinners." Correction: Hematuria on anticoagulants carries a similar rate of underlying pathology (stones, cancer) as the general population; a full urologic workup and CT Urogram are required.
  • Cognitive Trap (Penetrating Trauma Screening): Using a negative urinalysis to rule out renal or ureteral injury after a stab wound to the flank. Correction: The absence of hematuria is notoriously unreliable in penetrating trauma. Obtain an IV contrast-enhanced CT based purely on the anatomic proximity of the wound.
  • Critical Action: You must obtain an emergent Urology consultation for any patient presenting with a UTI, fever, and concurrent hydronephrosis (urinary obstruction). This combination rapidly leads to sepsis and renal necrosis, requiring immediate operative intervention.
