As bones displace, they shear the highly vascular mucosal linings of the sinuses and nasal cavity, resulting in profuse, life-threatening hemorrhage that rapidly compromises the airway.
- Neuromuscular & Sensory Compromise: Cranial nerves (e.g., V2, V3) traversing narrow bony foramina are stretched or severed, causing distinct facial paresthesias. Orbital floor blow-out fractures lead to herniation and entrapment of the inferior rectus muscle or orbital fat into the maxillary sinus, causing ischemia and restricted ocular movement.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Airway & C-Spine: Assume cervical spine injury and maintain immobilization. Evaluate for impending airway obstruction from blood, secretions, or loss of mandibular support. Prepare for early endotracheal intubation; have surgical airway backup ready for massive anatomical disruption.
- Hemorrhage Control:
- Action: Apply direct pressure to external wounds and perform early packing of the nasal and oral cavities.
- Contraindication: Strictly avoid blind clamping of bleeding vessels in the face to prevent permanent damage to the facial nerve or parotid duct.
- Globe Rupture Protocol: If a globe rupture is suspected, immediately place a rigid Fox eye shield. Keep the patient positioned head-up if spine cleared. Administer aggressive antiemetics and analgesia to prevent Valsalva-induced increases in intraocular pressure (IOP) that could cause extrusion of aqueous contents. Administer prophylactic antibiotics and order an emergent Ophthalmology consult. Do not manipulate the eye.

- Gold-Standard Imaging: Non-contrast Computed Tomography (CT) of the Maxillofacial bones, Head, and Cervical Spine is the definitive diagnostic modality.
- Vascular Imaging: CT Angiography (CTA) of the head and neck is often required to evaluate for blunt cerebrovascular injury, especially with cribriform plate or severe midface fractures.
- Specialist Consults: Activate a multidisciplinary team early (Neurosurgery takes first priority if intracranial involvement, followed by OMFS, ENT, Plastics, and Ophthalmology).
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- The Meticulous Visual Inspection:
- Lateral view: Look for a "dish face" deformity, indicative of a Le Fort III fracture.
- Frontal view: Look for a "donkey face" (elongation), indicative of Le Fort II or III fractures.
- Bird’s eye view: Look down from above for exophthalmos, highly suspicious for a retrobulbar hematoma.
- Worm’s view: Look up from below for enophthalmos, indicating a blow-out fracture or flattening of the malar eminence.

- A teardrop pupil pointing toward the injury indicates a globe rupture.
- Fat extruding through a wound indicates orbital septal perforation.
- Bilateral periorbital ecchymosis (raccoon eyes) or mastoid ecchymosis (Battle sign) indicates a basilar skull fracture.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- Le Fort Classification System: Used to risk-stratify midface instability.
- Le Fort I: Transmaxillary fracture (the hard palate separates from the maxilla).
- Le Fort II: Pyramidal fracture (the maxilla and nose separate from the orbits and zygoma).
- Le Fort III: Craniofacial dysjunction (the entire midface completely separates from the cranium; highest risk for airway compromise and associated brain injury).
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- The Tonometry Trap: Checking intraocular pressure (IOP) to rule out orbital compartment syndrome is indicated only if a globe rupture has been absolutely ruled out. Applying pressure to a ruptured globe will expel its contents.
- The Missed Septal Hematoma: Failing to inspect the nasal septum is a critical error. A septal hematoma must be urgently aspirated or incised and packed to prevent avascular necrosis of the cartilage and a permanent saddle-nose deformity.
- Premature Closure on Mechanism: Assaults, MVCs, and falls are common, but providers must always screen for domestic violence, elder abuse, and child abuse, as the vast majority of abused victims present with maxillofacial injuries.
- Critical Action: Follow strict ATLS protocols. Never let distracting, gruesome facial injuries delay the primary survey and identification of life-threatening thoracic, abdominal, or pelvic hemorrhage.
