- Unfractionated Heparin (UFH) & Low-Molecular-Weight Heparin (LMWH): Bind to antithrombin, accelerating its inactivation of Thrombin and Factor Xa. LMWH has predominantly anti-Xa activity compared to UFH.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
Determine if the patient has Major Bleeding (life-threatening, >5 g/dL drop in Hb, intracranial hemorrhage, or planned neurosurgery). If major bleeding is present, immediately discontinue the anticoagulant and initiate targeted reversal:
- Warfarin Reversal: Administer Vitamin K 5–10 mg IV PLUS 4-Factor Prothrombin Complex Concentrate (4F-PCC / Kcentra).
- PCC Dosing: Either a fixed dose (1,000–5,000 units) or INR-based (INR 2-<4 = 25 U/kg; INR 4-6 = 35 U/kg; INR >6 = 50 U/kg).
- Dabigatran Reversal: Administer Idarucizumab (Praxbind) 5g IV (given as two 2.5g doses no more than 15 minutes apart). If unavailable, use 4F-PCC 50 U/kg (max 5000 units).
- Rivaroxaban / Apixaban Reversal: Administer Andexanet Alfa (Andexxa). Dosing depends on the specific drug, dose, and timing:
- Low-Dose Protocol (400 mg IV bolus, then 480 mg infusion over 2 hours): Use if last dose of Rivaroxaban was $\le$10 mg, Apixaban $\le$5 mg, or if an unknown dose was taken $\ge$8 hours prior.
- High-Dose Protocol (800 mg bolus, then 960 mg infusion over 2 hours): Use if Rivaroxaban >10 mg, Apixaban >5 mg, or unknown dose taken <8 hours prior.

3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)
- PT/INR: The primary monitoring tool for Warfarin. The goal of emergency reversal is to normalize the INR rapidly.
- aPTT: Used to monitor UFH infusions. Prolongation can also be seen with Direct Thrombin Inhibitors.
- Anti-Xa Levels: Routine monitoring is not required for DOACs, but specific anti-Xa levels can be highly useful to guide reversal strategies or confirm the presence of LMWH/Factor Xa inhibitors prior to emergent surgery.
- Heparin-Induced Thrombocytopenia (HIT): Suspect HIT if a patient on heparin develops a >50% drop in platelets or a new thrombotic event (e.g., skin necrosis, DVT). Immediately discontinue all heparin products and initiate a non-heparin anticoagulant like Argatroban or Bivalirudin.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- Ischemic Stroke (Hyperdense MCA Sign): A bright, hyperdense middle cerebral artery on a non-contrast head CT indicates a large vessel occlusion (LVO) and acute thrombus, warranting immediate neurology/interventional consultation for thrombectomy.
- Massive Pulmonary Embolism (POCUS D-Sign): Bedside echocardiography revealing right ventricular dilatation (RV/LV ratio > 1.0) and septal bowing into the left ventricle (creating a "D-shaped" left ventricle) indicates catastrophic right heart strain.
- Aortic Dissection (Contraindication to Lytics): POCUS or CTA identifying an intimal flap in the aorta is an absolute contraindication to antithrombotics and fibrinolytics.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- HAS-BLED Score: A validated tool used to predict the 1-year risk of major bleeding in patients with atrial fibrillation on anticoagulation. It evaluates Hypertension, Abnormal renal/liver function, Stroke, Bleeding history, Labile INR, Elderly, and Drugs/alcohol. A score $\ge$3 indicates high bleeding risk.
- 4Ts Score for HIT (Outside Information): Evaluates Thrombocytopenia (degree of drop), Timing (onset 5-10 days post-exposure), Thrombosis (new clot), and oTher causes. - Deadly Pitfall (Systemic Fibrinolysis in Stroke): Administering systemic fibrinolytics (e.g., tPA/Alteplase) to an acute ischemic stroke patient who is taking any oral anticoagulant (DOAC or Warfarin) in the 3- to 4.5-hour extended window is an absolute contraindication, regardless of their current INR.
- Deadly Pitfall (Vitamin K Anaphylaxis): Administering IV Vitamin K via rapid push. It must be administered via a slow infusion (over 20 to 60 minutes) to prevent life-threatening anaphylaxis.
- Deadly Pitfall (LMWH Reversal): Relying on Protamine Sulfate to completely reverse LMWH. Protamine only partially neutralizes LMWH (approximately 50% effective).
- Critical Action (4F-PCC vs. FFP): In life-threatening bleeds (e.g., intracranial hemorrhage) on Warfarin, 4F-PCC is strongly preferred over Fresh Frozen Plasma (FFP). FFP requires 1 to 2 Liters of volume (high risk of fluid overload/TACO in the elderly), takes longer to thaw/administer, and corrects the INR slower than the low-volume, rapid-acting 4F-PCC.
