- Resuscitation for Undifferentiated Shock: In patients with systemic rheumatic diseases presenting with shock (especially those on chronic glucocorticoids like prednisone), rapid stabilization requires empirically treating for sepsis, hemorrhagic shock, obstructive pulmonary embolism, and adrenal crisis.
- Targeted Analgesia: For simple inflammatory arthritis flares, systemic analgesics—specifically anti-inflammatory medications (NSAIDs)—are the mainstay of initial ED therapy. Intra-articular corticosteroid injections may be considered in select populations where systemic therapy presents a significant risk or is contraindicated.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

Prioritized Diagnostic Workup:
- Arthrocentesis: Perform diagnostic joint aspiration any time infection is suspected or the diagnosis of an acute joint is unclear.
- Cardiac Screening: Have an incredibly low threshold to obtain an ECG and cardiac biomarkers in young patients with SLE or RA presenting with chest pain, as they carry up to a 10-fold increased relative risk for MI compared to healthy counterparts.
- Synovial Fluid Analysis: Order cell counts and cultures, but do not rely on the Gram stain in isolation to exclude infection.

- Standard ECG: Look for acute ischemic changes (STEMI/NSTEMI patterns) or signs of pericarditis. You must actively look for ischemia even in young, premenopausal women with lupus, as the baseline risk of MI is drastically elevated despite the absence of traditional risk factors.

- The "Crystal Exclusion" Trap: Finding crystals in a joint aspirate and assuming the joint is safe. Crystal arthritis and septic arthritis frequently coexist; the presence of crystals should never be used to exclude a concurrent joint infection.
- The "Negative Gram Stain" Trap: Relying on a negative synovial fluid Gram stain to rule out septic arthritis. The Gram stain is positive in only 50% of patients with a confirmed joint infection.
