  • Regional Anesthesia: To facilitate thorough wound exploration, foreign body removal, or management of severe pain (such as plantar frostbite or calcaneal fractures), perform an ultrasound-guided posterior tibial nerve block. This provides excellent anesthesia to the plantar surface of the foot without distorting the local tissue architecture.
  • Tendon Injury Stabilization: For suspected plantaris or Achilles injuries, place the patient in a posterior splint in a partial equinus (plantarflexed) position to reduce tension on the tendon. Provide strict return precautions for compartment syndrome and deep vein thrombosis (DVT).

3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

The Critical "Can't-Miss" Differentials:

  1. Retained Foreign Body: The leading cause of delayed necrotizing fasciitis or gangrene following a seemingly benign puncture wound.

Prioritized Diagnostic Workup:

  • Vascular & Perfusion Assessment: Palpate pedal pulses and assess capillary refill. A normal capillary refill of < 2 seconds and absence of "sausage digit" swelling are reassuring signs for adequate perfusion.
  • Footwear Inspection: You must actively examine the patient's footwear. A puncture occurring through the rubber sole of an athletic shoe is a critical historical clue for specific bacterial inoculation.

  • Bedside POCUS: Utilize a high-frequency linear probe to scan the soft tissues of the foot. Look for hyperechoic structures with posterior acoustic shadowing or reverberation artifact, which confirm a retained radiolucent foreign body. POCUS is also vital for identifying localized fluid collections or abscesses surrounding the puncture tract.
  • Plain Radiography (X-Ray): Scrutinize standard views for soft tissue gas (indicating necrotizing infection) or radiopaque debris. If the patient sustained high-impact trauma to the heel, obtain a lateral view to measure Boehler's angle. The normal angle is 25 to 40 degrees; an angle <25 degrees suggests a calcaneal fracture.

  • The Radiolucent Trap: Assuming a puncture wound is "clean" because plain X-rays are negative. Wood, thorns, and some plastics are radiolucent. Missed retained foreign bodies in the lower extremity frequently lead to severe cellulitis, deep abscesses, and necrotizing fasciitis.
  • The Bedside Extraction Trap: Attempting to aggressively dig out a deep plantar foreign body in the ED. Deep extraction is highly difficult, risks iatrogenic neurovascular injury, and should be deferred to a surgeon in the operating room.
