2. THE BEDSIDE ACTION PLAN (Rapid ER Management)

- Nonpurulent Cellulitis: Cephalexin 500 mg PO QID for 5–10 days (use Clindamycin 450 mg PO TID if severe penicillin allergy exists).
- Purulent Cellulitis/Abscess: Incision and drainage (I&D) is the definitive treatment. For adjunctive oral coverage (if >5cm, multiple lesions, or comorbidities exist), use Trimethoprim-Sulfamethoxazole (TMP-SMX) 1-2 DS tabs PO BID or Doxycycline 100 mg PO BID.
- Inpatient/Severe Management (Sepsis/NASTI):
- Initiate aggressive critical care fluid resuscitation.
- Administer broad-spectrum empiric antibiotics: Vancomycin (15–20 mg/kg IV) plus Piperacillin/Tazobactam (Zosyn) or a Carbapenem (e.g., Meropenem).

- Gold-Standard Imaging: Point-of-Care Ultrasound (POCUS) is the initial modality of choice to differentiate simple cellulitis from an occult abscess. CT or MRI is indicated to assess for deep-space infections or necrotizing fasciitis.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- The POCUS View: Utilize a high-frequency linear probe. An abscess will visually present as an anechoic or hypoechoic fluid collection. - The Imaging Delay: Pitfall: Delaying surgical debridement of a suspected necrotizing fasciitis while waiting for an MRI or CT scan.
