- Necrotizing Otitis Externa (NOE): A life-threatening, progressive infection of the external auditory canal and skull base, typically caused by Pseudomonas aeruginosa in elderly, diabetic, or immunocompromised patients. Presents with severe ear pain and granulation tissue in the external auditory canal; as it erodes the skull base, it can cause CN VII palsy. Deadly Cognitive Traps & Trainee Errors
- The "Frontalis Sparing" Stroke Trap: Mistaking an upper motor neuron (UMN) facial palsy (stroke) for a peripheral zoster palsy. If the patient can wrinkle their forehead and raise both eyebrows equally, this is frontalis muscle sparing and indicates a central lesion. Trainees must never diagnose Bell's palsy or Ramsay Hunt in a patient with frontalis sparing; doing so represents a catastrophic diagnostic failure.
- The Occult Canal Vesicle Miss: Diagnosing a patient with "Bell's Palsy" because they have facial weakness, without performing a comprehensive otoscopic exam. I will perform a detailed otoscopic examination of her right ear. I am looking for classic erythematous, painful vesicular clusters within the external auditory canal, concha, pinna, or on the tympanic membrane. I will also inspect her hard and soft palate for zoster vesicles.
To optimize her recovery, I will immediately initiate combined antiviral and systemic corticosteroid therapy.
