2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Immediate Stabilization: While isolated ocular infections are generally not airway threats, if the patient is systemically ill, the infection has likely progressed to sepsis or meningitis. Treat orbital-cellulitis-associated sepsis aggressively with intravenous fluids, broad-spectrum antibiotics, and vasopressors as needed.
- Intraocular Pressure (IOP) Management: Assess for ocular compartment syndrome. An IOP >20 mm Hg may be a surgical emergency. If elevated, you must add IOP-lowering medications (e.g., topical beta-blockers/alpha-agonists (External Knowledge)) to decrease pressure before surgical decompression in the ED.

- Orbital Cellulitis: Immediate initiation of broad-spectrum IV antibiotics to cover MRSA and anaerobes (e.g., Vancomycin 15-20 mg/kg IV + Ceftriaxone 2g IV or Ampicillin-Sulbactam 3g IV).
- Preseptal Cellulitis: Often managed with oral antibiotics covering Staphylococcus and Streptococcus (e.g., Trimethoprim-Sulfamethoxazole + Amoxicillin/Clavulanate) depending on local MRSA prevalence.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

- Cavernous Sinus Thrombosis: A lethal complication presenting with bilateral eye involvement, cranial nerve palsies (III, IV, V, VI), and profound systemic toxicity .
- Endophthalmitis: A deep intraocular infection (often post-surgical or post-traumatic) rather than an extraocular soft-tissue infection.
- Allergic Reaction / Contact Dermatitis: Presents with bilateral or unilateral painless, itchy swelling without fever or restriction of eye movement .
Prioritized Diagnostic Workup:
- The Clinical Exam: The initial differentiation between preseptal and orbital cellulitis is strictly clinical, based on history and physical exam. Remember, statistically, undifferentiated cases are far more likely to be preseptal (87%–94%).
- Gold-Standard Imaging: You must confirm orbital involvement and rule out complications with either an Axial and Coronal CT with contrast of the orbits and sinuses or an MRI with contrast.
- Laboratory Panels: Obtain blood cultures prior to antibiotic administration for any patient with suspected orbital cellulitis. Consider a lumbar puncture (LP) if concomitant meningitis is suspected.

- Preseptal Cellulitis: Characterized exclusively by eyelid swelling, redness and warmth of the skin overlying the orbit, skin tenderness, and conjunctival chemosis/palpebral injection. It is definitively differentiated from orbital cellulitis by the absence of deep orbital signs.
- Orbital Cellulitis: Look for the "Red Flags" of post-septal involvement: proptosis, pain with extraocular movements (EOM), ophthalmoplegia, and decreased visual acuity .
- The CT / MRI Checklist: When reviewing the contrast-enhanced CT of the orbits and sinuses, actively look for: a retained foreign body, retrobulbar abscess, orbital gas, subperiosteal abscess, associated osteomyelitis, and thrombotic changes in the cavernous sinus.

- Clinical Disposition Rules:
- Orbital Cellulitis: This is a definitive vision-threatening illness. The strict disposition rule is: Admit all cases of orbital cellulitis. These require emergent ophthalmology and often ENT consultation for possible surgical intervention to prevent serious CNS sequelae.
- Preseptal Cellulitis: Reliable adult patients with isolated preseptal cellulitis who are non-toxic, have a completely normal visual and motor exam, and can tolerate oral medications may be discharged with close 24-hour follow-up .

- The Superficial Trap: Diagnosing a patient with simple "preseptal cellulitis" without physically prying open the swollen eyelids to formally test visual acuity and extraocular movements. If you cannot visualize the globe to confirm normal EOMs and vision, you must assume orbital cellulitis and obtain a contrast CT.
- The Source-Control Pitfall: Failing to recognize that orbital cellulitis is almost always a secondary complication of ethmoid sinusitis. Focusing only on the eye and failing to consult ENT for sinus drainage can lead to refractory infection and vision loss .
- Critical Action: You must document the absence of proptosis, the presence of full/painless extraocular movements, and normal visual acuity to legally and medically justify discharging a patient with a "swollen red eye" as simple preseptal cellulitis.
