- Thrombin (Coagulopathy): An uncommon primary cause but a frequent secondary complication of severe hemorrhage, preeclampsia, sepsis, or amniotic fluid embolism. Suspect in patients with persistent bleeding from IV venipuncture sites, surgical wounds, or mucosal surfaces.
Prioritized Emergency Workup Strategy:
- Quantitative Blood Loss (QBL): Do not rely on visual estimations, which are notoriously inaccurate. Track blood loss volume and monitor for clinical signs of hypovolemia.

1. Clinical Definition of Postpartum Hemorrhage (ACOG 2017):
The diagnosis of postpartum hemorrhage is established when either of the following criteria is met within 24 hours of birth, regardless of the delivery route:
- Cumulative blood loss \(\ge\) 1,000 mL.
- Vaginal bleeding associated with any signs or symptoms of maternal hypovolemia (such as tachycardia, hypotension, dizziness, or poor peripheral perfusion).

4. There is guaranteed outpatient follow-up with obstetrics within 24 to 48 hours.
5. The patient is provided with strict return precautions (e.g., saturating **more than 2 sanitary pads per hour**, fever, or worsening pelvic pain).

Deadly Cognitive Traps & Trainee Errors:
- The "Normal Blood Pressure" Reassurance: Falsely assuming a postpartum bleeding patient is stable because her blood pressure is normal. Healthy young obstetric patients will compensate hemodynamically until they have lost \(\ge\) 1,500 mL of blood, after which they will rapidly collapse into profound hemorrhagic shock.
- The Vaginal Packing Trap: Attempting to pack the vaginal vault with gauze in a patient with active postpartum hemorrhage.
