  3. Vasa Previa: Fetal vessels traversing the membranes over the internal os. Classically presents as painless vaginal bleeding immediately following membrane rupture, rapidly progressing to fetal bradycardia and demise (as the blood loss is entirely fetal).
  4. Idiopathic Preterm Labor: Up to 10% to 20% of placental abruptions present initially as unexplained, painful preterm contractions without visible external bleeding.

Prioritized Diagnostic Workup Strategy:

  • Complete Blood Count (CBC) & Coagulation Studies: Monitor hemoglobin, platelets, PT, aPTT, and quantitative fibrinogen levels to identify early consumptive coagulopathy.
  • Basic Metabolic Panel (BMP): Baseline renal function must be evaluated, as severe hypovolemia and coagulation product accumulation in abruption can precipitate acute tubular necrosis or acute cortical necrosis.

5. THE SCORING MATRIX (Risk Stratification & Guidelines)

1. Clinical Severity Risk Categories

  • Mild Abruption: Mild uterine tenderness, mild or no vaginal bleeding, normal maternal vital signs, no maternal coagulopathy, and a reassuring fetal heart tracing.
  • Severe Abruption: Severe abdominal/uterine pain, continuous or repetitive uterine contractions, no or heavy vaginal bleeding, maternal hypotension or shock, maternal coagulopathy (depleted fibrinogen/DIC), or non-reassuring fetal heart tones/fetal demise.

2. Post-Trauma Cardiotocographic Monitoring Rules (ACOG/Trauma Guidelines)

All pregnant trauma patients \(\ge\)20 weeks' gestation must be evaluated with continuous cardiotocographic monitoring in the ED as soon as maternal stabilization is achieved.

  • The 4-Hour Rule: A minimum of 4 hours of continuous external cardiotocographic monitoring is mandatory for all stable pregnant trauma patients \(\ge\)20 weeks' gestation.
  • The Contraction Thresholds:

6. THE DANGER ZONE (Pitfalls & Critical Actions)

Deadly Cognitive Traps & Trainee Errors:

  • The Normal Ultrasound Reassurance: Discharging a symptomatic pregnant patient (e.g., following minor trauma or presenting with abdominal pain) because the bedside or formal ultrasound did not show a retroplacental hematoma. Ultrasound has an extremely poor sensitivity (~24%) for abruption; a negative scan means nothing.
  • The "Concealed Hemorrhage" Blind Spot: Assuming a patient with mild or absent vaginal bleeding is stable. In up to 20% of cases, the abruption is completely concealed (the margins of detachment remain intact, trapping up to liters of maternal blood behind the placenta).
