- Impaired Hemostasis: Unlike the thick, muscular myometrium of the uterine fundus, the lower uterine segment has poor contractile capacity. Consequently, once the vessels are torn, the uterus cannot perform the natural mechanical hemostasis of compressing the spiral arteries through myometrial contraction. This predisposes the patient to rapid, large-volume bleeding.

2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
Resuscitation of the mother is paramount and always takes absolute priority, as maternal stabilization is the single most effective way to optimize fetal perfusion.
- Step 1: Relieve Aortocaval Compression: For any patient beyond 20 weeks’ gestation, place the patient in a left lateral tilt of 15 to 30 degrees or perform manual leftward uterine displacement to decompress the inferior vena cava and restore venous return.
- Step 2: Aggressive Large-Bore Access: Establish at least two large-bore peripheral IV lines (14G or 16G), preferentially placed above the diaphragm.
- Step 3: Balanced Volume & Blood Resuscitation:
- Initiate rapid volume expansion with warm isotonic crystalloids (e.g., Lactated Ringer’s or 0.9% NaCl).
- Obtain an urgent Type and Crossmatch. If maternal hemodynamic stability is not rapidly achieved after 2 to 4 units of packed red blood cells (PRBCs), immediately activate the Massive Transfusion Protocol (MTP).
- Transfuse blood products in a balanced 1:1:1 ratio (PRBCs, fresh frozen plasma [FFP], and platelets). Consider prioritizing cryoprecipitate over FFP to correct hypofibrinogenemia (target fibrinogen >200 mg/dL) while minimizing volume overload.
- Step 4: Consider Tranexamic Acid (TXA): In cases of severe, ongoing hemorrhage, administer TXA 1,000 mg IV over 10 minutes (may repeat once within 24 hours). Because TXA crosses the placenta, its use should be discussed with the obstetric team.
- Step 5: Prevent Rh Alloimmunization: For all Rh-negative patients with bleeding, administer Rh(D) immune globulin (RhoGAM) 300 μg IM within 72 hours of symptom onset (protects against up to 30 mL of fetal blood). Obtain a Kleihauer-Betke test to quantify fetomaternal hemorrhage and determine if additional RhoGAM doses are required.
- Step 6: Antenatal Fetal Support:

Abruption is essentially a clinical diagnosis, as ultrasound has extremely poor sensitivity (~24%) to exclude it.
- Vasa Previa: Fetal blood vessels traversing the membranes overlying the internal cervical os. Classically presents as a lethal triad of membrane rupture, painless vaginal bleeding, and immediate fetal bradycardia or demise, as the bleeding is of fetal origin.
- Uterine Rupture: Complete tearing of the uterine wall, highly prevalent in patients with prior uterine scars (cesarean delivery, myomectomy). Presents with severe abdominal pain, vaginal bleeding, loss of uterine tone, palpable fetal parts outside the uterus, and maternal shock.
- Vaginal or Cervical Trauma: Lacerations or lesions of the lower genital tract.
Prioritized Diagnostic Workup Strategy:
- Emergency Bedside Ultrasound: Must be performed immediately to locate the placenta before any physical vaginal exam is attempted.
- Complete Blood Count (CBC): To evaluate baseline hemoglobin, hematocrit, and platelet levels.
- Coagulation Profile & Fibrinogen: Essential to evaluate for a consumptive coagulopathy, especially if there is suspicion of a concurrent concealed abruption.
- Continuous Fetal Cardiotocographic Monitoring: Initiate immediately in the ED once maternal resuscitation allows, to evaluate for fetal distress (such as late decelerations) and uterine contractions.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- Standard 12-Lead ECG Checklist:
- Identify normal physiologic changes of late pregnancy that mimic ischemia, such as left axis deviation (due to upward diaphragmatic displacement of the heart), prominent Q waves in inferior leads (II, III, aVF), and flattened or inverted T waves in precordial leads.
- Point-of-Care Ultrasound (POCUS) / Echocardiography View:
- Transabdominal Pelvic Sweep: Place a low-frequency transducer in the sagittal plane over the lower abdomen. Identify the internal cervical os and trace the lower uterine segment.
- Placental Placement: Visualize the highly echogenic, homogeneous placental tissue. Confirm if the placenta completely covers, partially covers, or lies near the internal os.
- The 4-cm Safe Margin: Measure the distance from the nearest placental edge to the internal cervical os. If the placental edge is >4 cm from the internal os, a diagnosis of placenta previa is highly unlikely.
- Sonographic Pitfalls Checklist:
- Bladder Distension: An overly distended maternal bladder can compress the lower uterine segment, bringing the anterior and posterior walls together and falsely mimicking placenta previa. If the diagnosis is equivocal, repeat the scan after the patient empties her bladder.
- Local Contractions: Focal myometrial contractions in the lower uterine segment can mimic placental tissue. Re-evaluate the placental-cervical relationship after the contraction has ceased.
- Transvaginal Ultrasound (TVUS) Safety Rules: TVUS is highly accurate, but if the patient is actively bleeding, direct intracavitary TVUS probing can worsen hemorrhage. If TVUS is deemed necessary by the specialist, ensure the transducer tip remains at least 3 cm away from the cervix.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
Ultrasound-Based Anatomical Classification:
- Placenta Previa: Placental tissue extends over or completely covers the internal cervical os.
- Low-Lying Placenta: Placental edge is located within 2 cm of the internal cervical os.
- Low-Lying Previa Risk: Placentas located within 1 cm of the os are associated with high rates of hemorrhage and require planned cesarean delivery. Placentas located 1 to 2 cm from the os may be candidates for a closely monitored trial of vaginal labor.

- Inpatient Admission Mandate:
- Any pregnant patient >20 weeks presenting with active vaginal bleeding and suspected or confirmed placenta previa.
- Any patient with resolved bleeding but ongoing maternal or fetal hemodynamic instability, or persistent uterine contractions.
- Safe Outpatient Discharge Criteria:
- Strictly reserved for completely asymptomatic patients with an incidental sonographic diagnosis of placenta previa made during routine prenatal care.
- The patient must be placed on strict home instructions: complete pelvic rest (strictly avoid penetrative intercourse, vaginal exams, or any sexual activity leading to orgasm), avoid standing for >4 hours at a time, and avoid lifting more than 20 lbs or performing strenuous activity.
- Indications for Emergent Cesarean Delivery:
- Active labor.
- Non-reassuring fetal heart tracings (fetal distress) that are refractory to aggressive maternal resuscitation.
- Ongoing maternal hemorrhage with an inability to achieve maternal hemodynamic stability.
- Significant bleeding in a patient who is close to term (at or beyond 34 to 36 weeks of gestation).

Deadly Cognitive Traps & Trainee Errors:
- The Vaginal Examination Catastrophe: Performing a digital bimanual or speculum vaginal examination in a pregnant patient >20 weeks’ gestation presenting with vaginal bleeding without first confirming the placental location on ultrasound. Disruption of the cervical-placental junction by digital palpation can precipitate catastrophic, life-threatening maternal hemorrhage.
- The "Painless equals Stable" Illusion: Assuming that a patient with painless vaginal bleeding is hemodynamically stable and safe. These patients are at high risk of rapid progression to hemorrhagic shock, and young, healthy patients can maintain normal blood pressures until they have lost up to 25% to 30% of their total blood volume.
- The "Normal Ultrasound" Reassurance in Abruption: Reassuring a patient that she does not have a placental abruption because the ultrasound is normal. Ultrasound has very poor sensitivity for abruption; a negative scan does not exclude this life threat if clinical suspicion exists.
