Acute Otitis Externa (Swimmer's Ear): Simple acute otitis externa represents a diffuse inflammation and infection of the external auditory canal (EAC) [61, 254]. Pathogenesis begins when normal host defenses, including the protective skin barrier and slightly acidic, hydrophobic cerumen layer, are compromised [61, 276, 599]. Cerumen acts as a critical antimicrobial barrier; elevation of local pH or hyperhydration (frequent water exposure/swimming) leads to skin maceration and creates a microscopic portal of entry for invasive pathogens [61, 129, 276]. Mechanical microtrauma (e.g., from cotton-tipped applicators, scratching, or cerumen disimpaction) further breaches this epithelial layer [61, 129, 276].
Pathogens: Pseudomonas aeruginosa and Staphylococcus aureus are the most common causative organisms [9, 61, 129, 276, 578]. Other gram-negative rods, Staphylococcus epidermidis, and Enterobacteriaceae may also be isolated [9, 61, 129, 276]. Fungal etiologies (Otomycosis, typically Aspergillus or Candida) are less common in primary acute cases but occur in warm/humid climates, after prolonged topical antibiotic courses, or in immunocompromised hosts [11, 62, 129, 602].
Necrotizing Otitis Externa (NOE): Previously known as malignant otitis externa, this is an aggressive, life-threatening, invasive infection [66, 256, 579]. It begins as simple otitis externa within the EAC and subsequently spreads through the periauricular soft tissues, cartilage, and periosteum, ultimately invading the temporal bone and skull base, causing skull base osteomyelitis [66, 67, 256, 280, 611]. It classically occurs in elderly, diabetic, or severely immunocompromised (AIDS, post-transplant, or undergoing chemotherapy) patients [252, 259, 280, 362]. In diabetic patients, vascular insufficiency/microangiopathy leads to cartilaginous degeneration, and a higher cerumen pH impairs local defenses, facilitating tissue spread along normal ear anatomy conduits [67, 611].
Pathogens: Pseudomonas aeruginosa is isolated in >90% of cases [67, 280, 281]. Community-acquired Methicillin-resistant S. aureus (MRSA) now accounts for approximately 15% of cases [67]. Severe fungal pathogens (Aspergillus, Candida) can cause NOE in patients with profound immunosuppression or end-stage AIDS [67, 281]. Importantly, NOE is not an oncologic malignancy; histopathology demonstrates inflammatory granulation tissue without malignant cells [257, 281]. It carries a high 10% to 20% mortality rate, which increases significantly in the presence of cranial neuropathies [256, 281]. - THE BEDSIDE ACTION PLAN (Rapid ER Management)
Airway & Breathing: Highly unlikely to cause immediate airway or breathing compromise [252, 362]. If present, evaluate for a concurrent deep neck space infection or alternate systemic pathology [252, 362].
Circulation (Sepsis Resuscitation): Systemic infiltration of P. aeruginosa from NOE can cause severe septic shock [363]. For systemically ill or febrile patients, initiate standard resuscitation: place large-bore IV access, obtain blood cultures, and administer IV crystalloid boluses and broad-spectrum IV antipseudomonal empiric therapy [253, 290, 363]. Stringent glycemic control is vital to optimize outcomes in diabetic patients [274].
Aural Toilet (Canal Cleansing): Gentle removal of desquamated skin and purulent debris is mandatory to allow topical drops to contact the infected canal epithelium [63, 255, 279, 604]. Perform gentle suctioning under direct visualization or use a soft plastic curette [63, 261].
TM Perforation Caution: If tympanic membrane (TM) perforation is known, suspected, or cannot be visualized due to severe canal edema, avoid ear canal lavage/irrigation and all acidic, alcohol-containing, or ototoxic topical medications [255, 261, 262].
First-Line Bedside Medications & Target Dosages
Uncomplicated Acute Otitis Externa (Topical Monotherapy for 7-10 Days): Do not prescribe oral antibiotics for uncomplicated local OE [132, 279, 607].
Ciprofloxacin-dexamethasone (Ciprodex) 0.3%/0.1% suspension: Instill 4 drops in the affected ear(s) every 12 hours [263, 609]. Safe with perforated TM or tympanostomy tubes [65, 278, 609].
Ofloxacin 0.3% otic solution: Instill 5 drops (children <13 years) or 10 drops (adults and children ≥13 years) QD for 7 days [609]. Alternatively, instill 10 drops every 12 hours [264]. Safe with perforated TM or tympanostomy tubes (Otic drug of choice in non-intact TM) [65, 264].
Ciprofloxacin-hydrocortisone (Cipro HC) suspension: Instill 3 drops in the affected ear(s) every 12 hours [263]. Do NOT use with perforated TM or tympanostomy tubes [65].
Neomycin/Polymyxin B/Hydrocortisone (Cortisporin) suspension: Instill 3-4 drops in the affected ear(s) 3 to 4 times daily [265, 610]. Do NOT use if TM is perforated or non-visualized due to vestibular/auditory aminoglycoside ototoxicity [65, 255, 262, 579, 610]. Neomycin also has high rates of local contact/chemical allergic dermatitis [265, 610].
Ear Wick Placement: If the EAC is significantly edematous or swollen shut, gently insert a compressed cotton or hydroxycellulose ear wick 10-12 mm into the canal [260, 261, 262, 610]. Moisten with topical antibiotic-steroid drops to facilitate proximal drug delivery [262, 610]. Instruct the patient that the wick will fall out spontaneously as edema resolves, or arrange follow-up for removal in 1-2 days [262, 610, 611].
Pain Control: Otalgia in OE and NOE is severe and must be addressed aggressively [116, 266, 610].
Ibuprofen: 10 mg/kg PO every 6 to 8 hours as needed (maximum pediatric daily dose of 40 mg/kg) [266, 380]. More effective than acetaminophen for pediatric otalgia [380].
Acetaminophen: 10-15 mg/kg PO every 6 hours as needed (pediatric maximum 75 mg/kg/day, not to exceed 3,000 mg per day) [266, 380, 410].
Topical Lidocaine 2% aqueous drops: Instill 3 drops into the external auditory canal for rapid, short-term pain relief while awaiting systemic analgesics (for patients older than 3 years with an intact TM) [379, 410, 411].
Necrotizing Otitis Externa (Intravenous Resuscitation & Long-Term Therapy):
Ciprofloxacin: 400 mg IV every 8 hours (or 400 mg IV every 12 hours) [273, 363, 614]. May be transitioned to ciprofloxacin 750 mg PO BID or TID as an outpatient in early, mild infections with close follow-up [69, 216].
Cefepime: 2,000 mg (2 g) IV every 8 hours or every 12 hours [273, 364]. (Pediatric: 50 mg/kg IV every 12 hours, max 2,000 mg per dose) [169, 220, 356].

3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)
Critical "Can't-Miss" Mimics
Acute Mastoiditis: Complication of acute otitis media characterized by postauricular erythema, swelling, tenderness, and downward/outward protrusion of the auricle with obliteration of the postauricular crease [11, 13, 293, 295, 616].
Ramsay Hunt Syndrome (Herpes Zoster Oticus): Reactivation of varicella-zoster virus in the geniculate ganglion [7]. Presents as severe otalgia out of proportion to findings, unilateral lower motor neuron facial paralysis (CN VII), and herpetiform vesicles inside the EAC or on the pinna, soft palate, oral cavity, face, or neck [7, 36, 197, 463, 482]. Vesicles may follow facial weakness by several days, making it initially indistinguishable from Bell's Palsy [199].
Auricular Perichondritis: Bacterial infection of the cartilaginous pinna (most commonly caused by P. aeruginosa from trauma or helical ear piercings) [210, 212, 225]. It presents with a red, swollen, and exquisitely tender outer ear but is distinguished by classic sparing of the lobule (earlobe), which contains no cartilage [211, 213, 373]. OE can involve the lobule [213].
Squamous Cell Carcinoma of the EAC: A dangerous mimic that must be suspected in any patient diagnosed with "refractory" otitis externa that fails to improve after standard antibiotic therapy [270]. Confirmed via biopsy [270].
Erysipelas of the Face: An acute Group A streptococcal cellulitis presenting as an angry red, warm, indurated, raised, and sharply demarcated plaque [154, 510]. Differentiated from cellulitis by the Milian ear sign (complete involvement of the ear, as the ear lacks the deeper dermis tissue typically involved in cellulitis) [154].
Diagnostic Workup Strategy for Suspected NOE
Step 1: Local & Systemic Cultures: Send cultures of purulent ear drainage for both bacterial and fungal pathogens [270]. Obtain blood cultures prior to antibiotic administration if the patient is febrile, toxic, or septic [253, 381, 547].
Step 2: Basic Labs & Inflammatory Biomarkers: Order a CBC (assess for leukocytosis, though often normal in bone/local soft tissue infections) [149, 492], basic metabolic panel (monitor renal function for drug clearances) [44, 458], and an Erythrocyte Sedimentation Rate (ESR) or C-reactive protein (CRP), which are typically elevated [270, 613].
Step 3: Gold-Standard Emergency Department Imaging (CT Temporal Bone with contrast): Perform a high-resolution, contrast-enhanced CT scan of the temporal bones/skull base [271, 385, 613, 632]. CT is the reference standard in the ED to detect bony erosion, temporal bone demineralization, fat plane obliteration in the subtemporal area, or soft-tissue fluid collections [271, 283, 385, 580, 613]. Note: Bony erosion may not be visible on CT until at least one-third of the bone has been destroyed [283].
Step 4: Advanced Soft Tissue Imaging (MRI Head and IAC with gadolinium contrast): Superior to CT in delineating soft-tissue changes, bone marrow involvement, and identifying intracranial extension/complications (meningitis, brain abscess, dural/sigmoid venous sinus thrombosis) [182, 271, 272, 283]. Caveat: MRI has decreased sensitivity for early cortical bony changes [272].
Step 5: Scintigraphy (Nuclear Medicine Scans)
Technetium-99m medronate bone scanning: Highly sensitive for identifying bone involvement (temporal osteomyelitis) based on osteoblast/osteoclast activity [284, 385]. Pitfall: Stays positive during bone repair and cannot be used to track treatment resolution [284, 385].
Gallium citrate Ga 67 scintigraphy: Excellent diagnostic tool that clears as infection resolves, making it the modality of choice to monitor for complete resolution of NOE [284, 386].
4. THE VISUAL BOARD (Imaging, POCUS, & ECG Findings)
Otoscopy Visual Checklist (Acute OE)
Edema & Erythema: EAC is diffusely red and swollen, occasionally swollen completely shut [62, 260].
Exudative Debris: Yellow, brown, white, or gray purulent canal debris [62, 600].
Tympanic Membrane Integrity: Look for visible tears, purulent otorrhea, or myringotomy tubes [114, 235]. If the TM is obscured by severe edema or debris, assume perforation and utilize non-ototoxic ciprofloxacin or ofloxacin drops [261].
Otoscopy Visual Checklist (NOE)
Granulation Tissue: Highly classic, pathognomonic erythematous granulation tissue visible on the floor of the external auditory canal at the bony-cartilaginous junction [68, 256, 269, 374, 612].
Otorrhea: Foul-smelling, purulent, yellowish-green discharge [256, 268, 269].
Pinna Prominence: The infected pinna is erythematous, edematous, and visibly pushed forward/outward compared to the unaffected side [68].
Cranial Neuropathy: Inspect facial symmetry to assess for cranial nerve VII (facial) palsy [68, 256, 269, 282, 612].
POCUS Soft Tissue Checklist (Cellulitis/Abscess Exclusion)
Using a high-frequency linear transducer (7.5-12 MHz) in two orthogonal planes, assess the periauricular soft tissues [532, 533]:
Cellulitis/Edema: Classic cobblestoning pattern (hyperechoic fat lobules separated by hypoechoic fluid) [152, 533].
Abscess: Irregular, well-defined, hypoechoic-to-anechoic fluid collections with posterior acoustic enhancement [152, 533].
Fistula/Necrotizing Soft Tissue Spread: Presence of subcutaneous gas (reverberation artifacts with dirty shadowing) or fluid tracking along deep fascial planes [4, 533].
Temporal Bone CT Visual Checklist (NOE Confirmation)
Fat Plane Obliteration: Obliteration of the normal subtemporal fat planes [283].
Bony Demineralization & Erosion: Destruction of the temporal bone, cortical irregularity, or erosion of the bony skull base [271, 283, 385, 580].
Mastoid Opacification: Opacification of mastoid air cells with fluid/thickened mucosa and loss of bony septal definitions (concomitant mastoiditis) [298, 386].
Sinus Thrombosis: On CT venography, look for a filling defect in the sigmoid or lateral dural venous sinus (sigmoid sinus thrombosis) [273, 283, 296, 307].
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
Diagnostic Criteria for Simple Otitis Externa: Established by the American Academy of Otolaryngology–Head and Neck Surgery Foundation [131, 136, 600]. A clinical diagnosis requires rapid onset (within 48 hours) in the past 3 weeks of:
At least one primary symptom: Otalgia (often severe), canal itching, or ear fullness [131, 136, 600].
AND at least one primary sign: Exquisite tenderness of the tragus or pinna, or diffuse ear canal edema/erythema [131, 136, 600].
NOE Risk Stratification & Mandatory Admission Criteria: There is no single eponymous scoring calculator (such as HEART or PERC) for NOE [613]. Rather, disposition is dictated by clinical host factors and red flags [252, 259, 362, 579]:

6. THE DANGER ZONE (Pitfalls & Critical Actions)
The Deadly Diagnostic Pitfall — "Refractory Swimmer's Ear": A classic, devastating cognitive trap (premature closure) is repeatedly discharging an elderly, diabetic, or immunocompromised patient with "resistant otitis externa" and prescribing multiple different courses of topical eardrops [68, 278, 281]. Board Pearl: Any patient with persistent ear pain and drainage despite 2 to 3 weeks of topical therapy has Necrotizing Otitis Externa until proven otherwise and requires immediate CT imaging and systemic therapy [68, 281].
The Iatrogenic Ototoxicity Trap: Prescribing topical aminoglycosides (Neomycin-Polymyxin-Hydrocortisone / Cortisporin) or acidic solutions (2% acetic acid) when the tympanic membrane is perforated, cannot be fully visualized, or in a patient with tympanostomy tubes [15, 63, 65, 255, 262]. This can cause permanent sensorineural hearing loss and vestibular damage [15, 63, 262, 331, 579, 610].
The Ramsay Hunt Misdiagnosis: Diagnosing Ramsay Hunt syndrome as simple Bell's Palsy [199]. Ramsay Hunt has a significantly worse prognosis, a lower rate of complete facial nerve recovery, and a risk of permanent sensorineural hearing loss [463].
