  • The Vital Sign of the Eye: The mandatory, absolute first step before any drops, lights, or interventions is to assess and document visual acuity.
  • First-Line Medications: Once other emergent causes are ruled out, the definitive treatment is high-dose corticosteroids to accelerate visual recovery.
  • Dosage: Administer Methylprednisolone 500–1,000 mg daily (can be given IV or orally) for 3–5 days.
  • Caveat: If the patient is already taking Disease-Modifying Therapies (DMTs) for known MS (e.g., IVIG, monoclonal antibodies, interferons), you must discuss with Neurology before prescribing high-dose steroids, as compounding immunosuppression can be dangerous.

3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

Top "Can't-Miss" Differentials:

  • Giant Cell Arteritis (Temporal Arteritis): A critical mimic in patients over 50. Causes painful vision loss but features headache, jaw claudication, and a high ESR/CRP. Missing this leads to irreversible contralateral blindness.
  • Central Retinal Artery Occlusion (CRAO): Presents with sudden monocular vision loss, but it is typically painless and features a classic "cherry-red spot" on the macula.
  • Retinal Detachment: Sudden, painless vision loss classically preceded by a sensation of "flashers and floaters" or a "curtain" coming down over the visual field.
  • Acute Angle-Closure Glaucoma: Painful vision loss with "halos around lights," but accompanied by a heavily injected (red) eye, a fixed mid-dilated pupil, vomiting, and highly elevated intraocular pressure.

Prioritized Diagnostic Workup:

  • Ocular Exam: Visual acuity testing, pupillary light reflex testing, and fundoscopy.
  • Gold-Standard Imaging: Magnetic Resonance Imaging (MRI) of the brain and orbits with gadolinium is highly sensitive for identifying optic nerve inflammation and asymptomatic demyelinating brain lesions.
  • Lumbar Puncture (LP): Useful if MS is suspected or to rule out CNS infection. Classic MS findings include a normal white cell count, elevated protein, elevated IgG, kappa light chains, and oligoclonal bands.

  • Fundoscopy: The examiner should evaluate the optic disc. In about one-third of cases, swelling of the optic disc (papillitis/papilledema) is visible. However, in two-thirds of cases, the inflammation is entirely retrobulbar (behind the globe), resulting in a completely normal-appearing retina .
  • MRI Brain/Orbits: Look for enhancement and thickening of the optic nerve on fat-suppressed contrast images , and evaluate the periventricular white matter for classic demyelinating plaques.

5. THE SCORING MATRIX (Risk Stratification & Guidelines)

  • McDonald Criteria: The definitive criteria used to diagnose Multiple Sclerosis. It requires proof of one or more lesions in 2 out of 4 central nervous system areas and dissemination in time (lesions occurring at different times).
  • Disposition Criteria: Optic neuritis alone is typically not an airway or hemodynamic threat. However, the patient must be referred urgently to Neurology. Admission to an observation unit or inpatient floor is indicated if the patient requires expedited MRI, IV steroid loading, or has significant concomitant neurological deficits (e.g., inability to ambulate, dysphagia, or severe ataxia).

6. THE DANGER ZONE (Pitfalls & Critical Actions)

  • The "Normal Exam" Trap: Prematurely discharging a patient with acute vision loss because the fundoscopic and external eye exam appears perfectly normal. Remember the optic neuritis maxim: "The patient sees nothing, and the doctor sees nothing" due to retrobulbar inflammation .
  • The Steroid Trap : A classic historical pitfall from the Optic Neuritis Treatment Trial (ONTT). You must never treat acute optic neuritis with standard low-dose oral prednisone (e.g., 60 mg), as this was shown to paradoxically increase the rate of recurrence. Treatment requires mega-dose steroids (500–1000 mg Methylprednisolone).
  • Critical Action: You must document the absence of an injected eye, normal extraocular structures, and a formal visual acuity test to legally separate this from surgical/traumatic ocular emergencies.
