- The Receptors: Opioids bind to mu, kappa, and delta receptors in the central nervous system. The primary life-threatening pathophysiology is mu-receptor agonism in the medullary respiratory centers, which drastically blunts the hypercapnic drive.

2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Airway & Breathing First: Airway protection, oxygenation, and ventilation are the absolute cornerstones of management. If the patient is apneic, initiate bag-valve-mask (BVM) ventilation with 100% oxygen before focusing on antidotes.
- The Reversal Agent (Naloxone): Administer intravenous (IV), intramuscular (IM), or intranasal naloxone.

- Active Withdrawal Management: For patients in active withdrawal, Buprenorphine is safe and highly recommended in the ED to reduce mortality. Administer an initial dose of 4 to 8 mg sublingually (SL). If buprenorphine is unavailable, clonidine can be used to suppress sympathetic hyperactivity.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)
- The "Can't-Miss" Mimics:
- Clonidine Toxicity: Presents nearly identically with miosis, bradycardia, hypotension, and lethargy.
- Sedative-Hypnotic/Benzodiazepine Overdose: Causes severe respiratory depression and coma, though typically presents with normal or mid-position pupils.
- Hypoglycemia: A rapidly reversible metabolic mimic of coma.
- Prioritized Diagnostic Workup:
- Co-ingestant Screening (Mandatory): Always draw quantitative serum acetaminophen and salicylate levels. Many prescription opioids are formulated as combination products (e.g., Percocet, Vicodin), and failing to check these levels can lead to missed hepatotoxicity.
- Serum Ethanol: Frequently co-ingested and compounds respiratory depression.
- Urine Drug Screens (UDS): Do NOT rely on UDS. A negative screen is notoriously unreliable (frequently missing synthetic fentanyl analogs) and should never deter a diagnosis of opioid intoxication when the clinical toxidrome is present.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- The 12-Lead ECG Checklist: Essential for identifying specific opioid complications. Look explicitly for QRS widening (a classic sign of propoxyphene/norpropoxyphene toxicity causing sodium channel blockade) and QTc prolongation (classic for methadone toxicity).
- Chest Radiograph (CXR): If you auscultate pulmonary rales or the patient remains hypoxic after reversal, obtain a portable CXR to evaluate for non-cardiogenic pulmonary edema (acute lung injury).
- The Physical Exam Visuals: Fully undress the patient. Actively search for hidden fentanyl patches on all body parts, including mucous cavities, and assess muscle groups for early signs of compartment syndrome resulting from prolonged down-time.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- The 4-Hour Observation Rule: Patients treated with naloxone for a presumed short-acting opioid overdose can be safely discharged after a 4-hour observation window if they remain asymptomatic with normal vital signs. Studies show delayed respiratory arrest after this 4-hour mark is exceptionally rare (0.1% - 0.6%).
- Clinical Opiate Withdrawal Scale (COWS): Used to stratify withdrawal severity and guide buprenorphine induction. A COWS score of $\ge$ 13 (Moderate-Severe) is the threshold to strongly consider starting with 8 mg of sublingual buprenorphine.

Critical Action: Simple opioid withdrawal causes severe agitation, restlessness, and autonomic signs, but it does not alter cognition. If a withdrawing patient is confused, you must actively search for another etiology.
- The Lomotil Trap: Pitfall: Discharging a toddler who ingested a single diphenoxylate/atropine (Lomotil) tablet because they look fine at 2 hours. Critical Action: Asymptomatic pediatric patients with known Lomotil ingestion must be admitted for prolonged observation due to the delayed onset of deadly toxicity from its active metabolite.
- The Antagonist Trap: Pitfall: Using Nalmefene instead of naloxone. Critical Action: Avoid nalmefene; it is a long-acting opioid antagonist that will plunge the patient into prolonged, untreatable withdrawal.
