Nontraumatic hand emergencies are primarily driven by closed-space infections that rapidly elevate tissue pressures, leading to microvascular compromise and structural necrosis. The fingertip pulp contains numerous dense fibrous septa that rigidly anchor the skin to the distal phalanx. When bacteria (most commonly Staphylococcus aureus) inoculate this space, the resulting purulent exudate cannot expand outward; instead, interstitial pressure rapidly exceeds capillary perfusion pressure, driving local ischemic necrosis and allowing the infection to directly seed the underlying bone, causing osteomyelitis. Similarly, the flexor tendon sheaths are closed synovial spaces with a tenuous blood supply; bacterial replication here triggers profound inflammation that can quickly destroy the tendon or spread proximally. Simple superficial infections, such as a paronychia of the lateral nail fold, can easily transgress these anatomical boundaries if left untreated, extending into the deeper pulp (felon) or the flexor tendon sheath (flexor tenosynovitis).
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Superficial Infections (Paronychia): For mild cases without fluctuance, manage with aggressive warm soaks and oral antibiotics (e.g., Cephalexin). If fluctuance is present, incision and drainage is the definitive mainstay of treatment.
- Deep Pulp Infections (Felon): Immediate incision and drainage is required to decompress the fibrous septa and prevent digital ischemia. Add adjunctive systemic antibiotics if there is surrounding cellulitis.
- Tendon Sheath Infections (Flexor Tenosynovitis): This is a surgical emergency. Do not attempt bedside drainage. You must immediately initiate intravenous antibiotics, restrict oral intake (NPO), and arrange admission to the hospital for possible operative drainage by a hand specialist.
- Pain & Function Control: Elevate the affected extremity to reduce throbbing dependent edema. For non-infectious mechanical entrapments like trigger finger (digit stuck in flexion), conservative management and specialist follow-up is indicated.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)
The Critical "Can't-Miss" Differentials:
- Acute Suppurative Flexor Tenosynovitis (Surgical emergency threatening permanent tendon necrosis).
- Felon (Threatens the distal phalanx with osteomyelitis and ischemic necrosis).
- Disseminated Gonococcal Infection (Must be considered in non-traumatic tenosynovitis).
- Trigger Finger (Non-infectious tendinopathy mimicking a locked/flexed digit).
Prioritized Diagnostic Workup:
- Clinical Phenotyping: The diagnosis of deep hand space infections is primarily clinical, relying heavily on the exact location of fluctuance and pain provocation.
- Neuromuscular Assessment: Perform an exact 2-point discrimination test at a highly specific 5 mm threshold to rule out compressive or ischemic neuropathy.
- Sexual History: If the patient has isolated flexor tenosynovitis without a clear history of penetrating trauma, you must actively screen for high-risk sexual behavior to rule out a disseminated gonococcal etiology.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- Bedside POCUS: Point-of-care ultrasound is highly valuable in differentiating a localized fluid collection (abscess requiring I&D) from diffuse cellulitis. It is also utilized to visualize a thickened tendon or nodule catching on the A1 pulley in cases of trigger finger.
- Motor Function Visual Checklist:
- Median Nerve: Instruct the patient to make an "OK sign" against resistance using the 1st and 2nd digits.
- Ulnar Nerve: Instruct the patient to perform a "Finger spread" against resistance.
- Radial Nerve: Instruct the patient to make a "Thumbs up" sign.
- Sensory Visual Checklist: Map the exact dermatomes via volar/dorsal testing.
- Median Nerve: Volar aspect of the 2nd digit.
- Ulnar Nerve: Volar or dorsal aspect of the 5th digit.
- Radial Nerve: Dorsal web space between the 1st and 2nd digits.

Kanavel's Cardinal Signs:
The absolute gold-standard diagnostic criteria for Acute Flexor Tenosynovitis. The presence of these signs dictates an immediate surgical consultation. The four classic signs are:
- Fusiform swelling (sausage-like swelling of the entire digit).
- Finger held in slight flexion at rest.
- Tenderness to palpation over the flexor tendon sheath (volar aspect).
- Disproportionate pain with passive extension of the digit.
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- The Spreading Infection Trap: Discharging a paronychia without explicit return precautions. Untreated lateral nail fold infections can easily dissect into the deeper structures, causing a felon or flexor tenosynovitis.
- The Gonorrhea Trap: Assuming all flexor tenosynovitis requires a penetrating puncture wound. If there is no clear history of trauma and the patient is sexually active, you must consider disseminated Neisseria gonorrhoeae.
- The Delayed Felon Trap: Failing to aggressively drain a felon early. Leaving the pulp under pressure leads to devastating complications including ischemic necrosis, osteomyelitis, flexor tenosynovitis, and septic arthritis.
- CRITICAL ACTION: If Kanavel's signs are present, surgical washout is necessary to prevent permanent joint and tendon damage; do not delay IV antibiotics or hand surgery consultation.
