Understanding the precise mechanism of injury and the estimated energy transferred is crucial for predicting orthopedic injury patterns and hidden trauma. High-kinetic-energy mechanisms, such as motor vehicle collisions or falls from significant heights, frequently result in comminuted, displaced, or open fractures with extensive soft tissue stripping. Conversely, rotational and twisting forces typically produce spiral fractures, while direct lateral blows cause transverse or oblique fractures. Mentally reconstructing the physical forces allows the emergency physician to anticipate life-threatening hemorrhage, occult fractures, and concurrent neurovascular injuries along the anatomical pathway.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Immediate Stabilization: Orthopedic trauma requires addressing the ABCs first; life-threatening hemorrhage from open wounds or mangled extremities must be immediately controlled with direct pressure or tourniquets.
- Pain Control & Sedation: Early administration of systemic analgesics (e.g., IV opioids) is mandatory. Procedural sedation provides critical muscle relaxation, facilitating easier reductions and reducing the risk of iatrogenic fracture or nerve entrapment. Regional anesthesia, such as a femoral nerve block, is an excellent, hemodynamically stable alternative for severe lower extremity trauma.
- Reduction & Splinting: Realign the fractured limb to its anatomical position to relieve tension on nerves and vessels. Immobilization must include the joints immediately above and below the fracture site.
- Open Fracture Protocol: For open fractures, remove gross debris, irrigate with saline, and cover with a sterile saline-soaked dressing without prematurely pushing exposed bone back into the wound. Immediately administer a tetanus booster and IV antibiotics; a first-generation cephalosporin is standard, with an added aminoglycoside for highly contaminated or severe (Type II/III) wounds. Do not swab open wounds for cultures in the ED.

The classic "5 Ps" (pain, pallor, paresthesias, paralysis, poikilothermia) may be present, but severe pain out of proportion to the injury or pain with passive stretch is the hallmark.
