  • Conductive Hearing Loss (CHL): Driven by a mechanical disruption in sound transmission through the external auditory canal or middle ear structures. The tympanic membrane (TM) is a thin sheet of connective tissue that transmits and amplifies sound waves to the ossicles. Perforation of the TM, cerumen impaction, foreign body obstruction, or ossicle dislocation prevents these sound waves from physically reaching the fluid-filled cochlea.
  • Sudden Sensorineural Hearing Loss (SSNHL): Characterized by an acute loss of at least 30 dB occurring over less than 3 days across at least three contiguous frequencies. Because of the terminal branching and interosseous location of the blood supply to the inner ear, the cochlea is uniquely vulnerable to vascular occlusion, hematologic diseases, and ischemia. Idiopathic in 70% of cases, cellular damage to the delicate hair cells within the organ of Corti can also be triggered by direct viral invasion (classically mumps or varicella-zoster), autoimmune-mediated cochlear membrane destruction, or mechanical compression of cranial nerve VIII by an intracranial mass (e.g., vestibular schwannoma).

If my examination reveals a normal ear canal, an intact tympanic membrane, and no focal neurological or central signs, I will diagnose idiopathic sudden sensorineural hearing loss. I will perform a meticulous medication reconciliation and immediately discontinue any potentially ototoxic agents.

To optimize auditory recovery, I will immediately initiate systemic corticosteroid therapy with oral prednisone 1 mg/kg/day up to a maximum dose of 60 mg daily, tapered over 10 to 14 days.
