  • Endocardial Invasion: Infective endocarditis (IE) is an infection of the native heart valves, prosthetic valves, the endocardium, or indwelling cardiac devices.

2. THE BEDSIDE ACTION PLAN (Rapid ER Management)

  • Immediate Resuscitation: Patients may present in extremis with hemodynamic instability, pulmonary edema, altered mental status, acidosis, and respiratory compromise. Immediately stabilize with airway management, positive pressure ventilation, and hemodynamic monitoring and support. Intra-aortic balloon counterpulsation may be used to aid hemodynamics in severe failure.
  • Sepsis Protocols: Initiate evidence-based therapy for sepsis simultaneously with cardiovascular support.
  • First-Line Pharmacotherapy: Administer intravenous Vancomycin. In the ED, empiric therapy should almost always include vancomycin. Vancomycin alone for one or two doses is a highly reasonable initial approach for both native and prosthetic valve infections.
  • Medications to AVOID: If the patient is in acute heart failure secondary to IE, there is strictly no role for initiating beta-blockers. Furthermore, heparin and digoxin are not indicated in the management of IE.

  • Acute Heart Failure from structural rupture (e.g., papillary muscle rupture post-MI).
  • Severe Pneumonia/Sepsis (right-sided IE showers the lungs, mimicking community-acquired pneumonia with rhonchi and hypoxia).
  • Acute stroke syndromes (can mimic the cerebral emboli of left-sided IE).
  • Prioritized Diagnostic Workup:
  • Blood Cultures: Obtain cultures immediately, as demonstrating microorganisms via culture is a core diagnostic criterion.
  • Echocardiography: Gold-standard imaging for detecting vegetations and evaluating valvular competence.
  • Advanced Neurologic Imaging: Obtain a brain MRI, even in patients without frank neurologic symptoms. Asymptomatic cerebral emboli are incredibly common in left-sided IE, and discovering them alters risk stratification.

  • POCUS (Echocardiography): Emergency clinicians must utilize point-of-care ultrasound to evaluate for left-sided or right-sided endocarditis. Look for oscillating masses (vegetations) on the valves, incomplete valve closure, and signs of acute regurgitation leading to secondary volume overload.
  • Brain MRI: Actively look for embolic infarcts. The presence of asymptomatic cerebral emboli on MRI is a specific diagnostic finding.

5. THE SCORING MATRIX (Risk Stratification & Guidelines)

  • Modified Duke Criteria for Infective Endocarditis: This is the validated clinical decision tool for diagnosing IE.
  • Definite IE (Pathologic Criteria): Requires microorganisms demonstrated by culture, or the histologic examination of a vegetation.
  • Minor Criteria Elements: The presence of asymptomatic cerebral emboli (found on brain MRI) officially constitutes a Duke minor criterion.

6. THE DANGER ZONE (Pitfalls & Critical Actions)

  • The Nonspecific Presentation Pitfall: Pitfall: Failing to consider IE because the patient lacks classic stigmata. Critical Action: Recognize that IE has a highly variable and nonspecific presentation. The only consistent feature is fever. Emergency clinicians must maintain a high index of suspicion.
  • The Heart Failure Mismanagement Trap: Pitfall: Attempting to medically manage a patient with IE who has developed acute heart failure with beta-blockers, digoxin, or heparin. Critical Action: None of these agents are indicated. Patients with suspected IE and acute heart failure require immediate consultation by a cardiothoracic surgeon, as nearly half of left-sided IE patients require surgery during the index admission.
  • The Transfer Trap: Pitfall: Admitting a patient with left-sided IE and heart failure to a community hospital without surgical capabilities. Critical Action: These patients must be managed by a multidisciplinary team at a center capable of cardiothoracic surgery.

Differentiation: These are distractors. The correct answer will always be to "Consult Cardiac Surgery" immediately, as mechanical valve failure requires surgical, not medical, resolution.
  • Antibiotic Selection: If asked for the best initial empiric antibiotic in the ED for suspected native or prosthetic valve IE, the answer is Vancomycin.

  • Communication Pearls: "Given the patient's highly variable presentation with a persistent fever and a new murmur, Infective Endocarditis is at the top of my differential. The presentation is nonspecific, so I will maintain a high index of suspicion.".
  • Articulating the Management Plan: "I am immediately initiating sepsis protocols and stabilizing the patient with positive pressure ventilation and hemodynamic support. I will draw blood cultures to satisfy Duke criteria and initiate empiric IV Vancomycin.".
