1. THE 2-MINUTE PHYSIOLOGY (Rapid Pathophysiology)
- The Core Mechanism: The physiologic changes that occur in pregnancy often exceed the patient’s underlying compensatory mechanisms, resulting in the rapid decompensation of pre-existing medical illnesses.
- Cardiovascular Overload: Pre-existing heart disease worsens dramatically because maternal blood volume and cardiac output significantly increase during gestation.
- Hypercoagulability: Pregnancy is a profoundly hypercoagulable state due to increased coagulation factors and venous stasis. The risk of venous thromboembolism (VTE) increases to five or six times that of nonpregnant women, with the highest risk occurring during the first six weeks postpartum.
- Metabolic & Endocrine Shifts: Endocrine demands shift drastically. Severe hypoglycemia is highly common in the first trimester. In cases of hyperemesis gravidarum, patients enter a state of starvation metabolism characterized by dehydration, weight loss, and prolonged ketonemia and ketonuria.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Hypertensive Emergency: Defined as acute-onset persistent hypertension with a systolic BP > 160 mm Hg or diastolic BP > 110 mm Hg lasting for > 15 minutes.
- Intervention: Administer therapy within 30 to 60 minutes of diagnosis. Target BP is 140–150 mm Hg systolic and 90–100 mm Hg diastolic.
- First-Line Agents: Labetalol 20 mg IV (repeat 40 mg in 10 min if refractory), Hydralazine 5 or 10 mg IV, or Nifedipine 10 mg PO.
- Eclamptic Seizures:
- First-Line: Magnesium Sulfate 4 g IV loading dose, followed by a 2 g/hr infusion. This prevents recurrent seizures while uniquely maintaining uterine and fetal blood flow.
- Refractory: If seizures continue, utilize benzodiazepines, followed by phenytoin, and levetiracetam.
- Fluid restriction: Limit IV fluids to 80 mL/hr or 1 mL/kg/hr to prevent pulmonary edema.
- Acute Asthma Exacerbation: Keep maternal oxygen saturation > 95% to prevent fetal hypoxia. Inhaled beta-agonists and corticosteroids are safe and remain first-line.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)
- "Can't-Miss" Mimics & Etiologies:
- Spontaneous Coronary Artery Dissection (SCAD): The most common cause of acute myocardial infarction (AMI) in pregnant women.
- Aortic Dissection: Must be considered in any pregnant patient presenting with unexplained severe chest, back, or neck pain.
- Acute Appendicitis: The most common surgical emergency in pregnancy. Presentation is often atypical, particularly in the second half of gestation due to uterine displacement.
- Prioritized Diagnostic Workup:
- Preeclampsia Panel: CBC, U&E, LFTs, uric acid, and clotting screen. Note: Proteinuria is no longer strictly required for the diagnosis of preeclampsia.
- Anemia & Infectious Workup: Serum ferritin is the most accurate lab value for diagnosing iron deficiency. If an HIV-infected mother presents with moderate to severe anemia, you must promptly initiate a workup for tuberculosis.
- Fetal Heart Rate: Utilize early fetal heart rate monitoring as an additional vital sign (normal is 120–160 bpm) to detect maternal/fetal distress.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- ECG Analysis: When evaluating ischemic chest pain, scrutinize for STEMI patterns but remember that the underlying pathophysiology is most likely spontaneous coronary artery dissection, not atherosclerotic plaque rupture.
- Radiography/CT: Do not withhold life-saving imaging. Protect the fetus when able, but prioritize the optimal resuscitation of the mother and obtain imaging as clinically indicated.
- Cardiotocographic Monitoring: In cases of mild blunt trauma, maternal vital signs or ultrasound results do not predict fetal outcome; a minimum of 4 hours of cardiotocographic monitoring is required to rule out occult fetal consequences.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- Hypertensive Emergency Criteria & Disposition:
- A BP of ≥ 160/110 mm Hg persistent for > 15 minutes is a true obstetrical emergency.
- If BP remains ≥ 160/110 mm Hg despite sequential doses of IV labetalol, hydralazine, or PO nifedipine, emergent specialist consultation (Maternal-Fetal Medicine, Anesthesia, Critical Care) is mandated.
- Hyperemesis Gravidarum Criteria: Diagnosis requires nausea and vomiting severe enough to cause starvation metabolism, weight loss, dehydration, and prolonged ketonemia and ketonuria.
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- Deadly Cognitive Trap (Thrombolytics in ACS): Administering fibrinolytics to a pregnant patient with an AMI can drastically worsen a spontaneous coronary artery dissection. Furthermore, thrombolytics preclude major surgery (cesarean) or epidural anesthesia in women close to term.
- Critical Action (Sickle Cell Pain Crises): While treated similarly to nonpregnant patients, Hydroxyurea is strictly contraindicated due to known teratogenicity.
- Critical Action (Syphilis Treatment): Penicillin is the only effective treatment for congenital syphilis. Pregnant patients with a penicillin allergy must be formally desensitized and treated with penicillin.
- Critical Action (Endocrine Therapies): Radioiodine is strongly contraindicated for the treatment of hyperthyroidism in pregnancy. Bisphosphonates are strictly contraindicated for the treatment of hypercalcemia.
7. MCQ MASTERCLASS (Written Exam Tips)
- The Antiemetic Distractor: A question describes severe nausea and vomiting in early pregnancy and asks for the best outpatient medication. Ondansetron is a common distractor. The correct first-line pharmacologic agent is Diclegis (doxylamine 10 mg and pyridoxine 10 mg).
- The Seizure Distractor: An actively seizing pregnant patient (eclampsia) requires an anticonvulsant. Distractors will include Labetalol or Hydralazine. The correct answer is Magnesium Sulfate because it stops the seizure without compromising uteroplacental blood flow; antihypertensives alone will not break an eclamptic seizure.
- The "Most Common" Buzzwords: The most common surgical emergency in pregnancy is Appendicitis. The most common cause of AMI in pregnant women is Spontaneous Coronary Artery Dissection.

- The Stabilization Hook: "Given the patient is 28 weeks pregnant with a persistent blood pressure of 165/115 mm Hg for over 15 minutes, she is in an obstetric hypertensive emergency. I will immediately administer Labetalol 20 mg IV, targeting a blood pressure of 140 to 150 systolic and 90 to 100 diastolic."
