4. THE VISUAL BOARD (ECG / POCUS / Imaging)

  • 12-Lead ECG expected changes:
  • Physiologic changes in normal pregnancy: Leftward shift of the mean QRS axis (averaging 15 degrees due to diaphragmatic elevation), flattened or inverted T waves in lead III and leads V1–V3, and prominent Q waves in inferior leads (II, III, aVF).
  • Pathologic changes (SCAD/AMI): Look for ST-segment elevations (STEMI), ST-segment depressions, hyperacute T waves, or a new left bundle branch block (LBBB).
  • Point-of-Care Ultrasound (POCUS):
  • Pulmonary Embolism: Evaluate for signs of acute right ventricular (RV) strain, including RV dilation (RV diameter \(>\) LV diameter), septal bowing into the left ventricle (the classic "D" sign on parasternal short-axis view), or McConnell's sign.

  • The "Radiation Phobia" Delay: Trainees frequently delay or withhold necessary CT angiography (CTPA or aortic CTA) out of concern for fetal radiation exposure. This is a critical error. The radiation dose from a CTA is well below the threshold for fetal harm, and maternal survival dictates fetal survival. Do not withhold essential imaging.
  • The ACS Thrombolysis Trap: Administering systemic thrombolytics empirically to a pregnant patient presenting with an apparent acute MI is highly dangerous. Because SCAD is the most common cause of AMI in pregnancy, thrombolytics can cause rapid propagation of the coronary intramural hematoma, worsen dissection, and precipitate catastrophic maternal or placental hemorrhage.
