2. THE TRIAGE & SEVERITY CRITERIA

- MINOR (Green / "Worried Well"): The vast majority of the surge. These are asymptomatic or mildly symptomatic individuals seeking prophylactic treatment or testing. To preserve hospital resources, these patients should be triaged to off-site, community-based prophylactic dispensing centers or designated for home observation.
- EXPECTANT (Black): Patients who are apneic and pulseless, or remain apneic after airway positioning (or after 5 rescue breaths in pediatrics). In severe resource-constrained or austere environments, these patients are triaged to palliative care.
3. THE IMMEDIATE THREAT PROTOCOL
- Step 1: Departmental Lockdown: Immediately restrict facility access and implement hospital lockdown procedures to control the influx of potentially contagious patients and the "worried well".
- Step 2: Universal PPE Upgrade: If a CBRNE (Chemical, Biologic, Radiologic, Nuclear, Explosive) event is suspected but the agent is unknown, providers must immediately initiate a conservative approach using high-level PPE—combining Standard, Contact, and Airborne precautions—until the pathogen is identified.
- Step 3: Isolation and Cohorting: Patients suspected of highly contagious diseases (e.g., smallpox or viral hemorrhagic fevers like Ebola) must be immediately placed in negative-pressure airborne isolation rooms, physically separated from the rest of the hospital population. 4. THE TOXICOLOGY / PATHOLOGY MATRIX
Class A agents pose the highest risk to national security and public health due to ease of dissemination and high mortality rates.
- Smallpox (Variola major): Incubation is 7–19 days. Patients develop a severe febrile prodrome, followed by a maculopapular rash. Diagnostic Marker: Unlike naturally occurring varicella, classic smallpox lesions are uniquely characterized by being in the exact same stage of development across the body.
- Inhalational Anthrax (Bacillus anthracis): Typically spread via aerosolized spores. Patients initially present with ILI symptoms. Diagnostic Marker: The classic progression includes pulmonary consolidation, pleural effusions, and a widened mediastinum visible on chest radiograph or CT.
- Botulinum Toxin: Toxin produced by Clostridium botulinum, presenting after inhalation of aerosolized agent. Diagnostic Marker: Acute, symmetric descending flaccid paralysis, diplopia, dysphagia, and dysarthria, with absolutely no change in mental status.
- Tularemia: Presents in 6 major clinical forms: ulcero-glandular, glandular, oculo-glandular, pharyngeal, pneumonic, and typhoidal.
- Pneumonic Plague (Yersinia pestis) (External Knowledge): A rapidly fatal, highly contagious pneumonia causing hemoptysis and overwhelming sepsis.
5. THE ANTIDOTE & THERAPEUTIC GRID
- Anthrax: First-line treatments most effective during the prodromal phase are Ciprofloxacin or Doxycycline. Contact precautions are only necessary if there is uncontained, copious drainage from a cutaneous eschar.
- Tularemia: Administer Streptomycin, Gentamicin, Doxycycline, or Ciprofloxacin. Person-to-person spread is rare; standard precautions are generally sufficient.
- Botulism: The toxin cannot be spread via droplet or contact; standard precautions are required. Administer botulinum antitoxin early (External Knowledge).
- Smallpox: Treatment is primarily supportive. Implement strict Standard, Contact, and Airborne precautions for up to 3–4 weeks until scabs heal completely. If possible, only immune healthcare workers should provide direct patient care.
- Pneumonic Plague: Requires strict Droplet precautions until the patient has received a full 48 hours of appropriate antibiotic therapy.
6. THE DANGER ZONE
