Acute arterial occlusion is a limb-threatening emergency resulting from the sudden cessation of blood flow to an extremity, leading to profound tissue hypoxia, anaerobic metabolism, and impending necrosis. The underlying pathophysiology is driven by one of two primary mechanisms, which dictate the clinical presentation:
- Arterial Embolism: The sudden impaction of a clot in a previously healthy, un-collateralized vessel. Approximately 85% of arterial emboli originate in the heart. - In Situ Thrombosis: The gradual occlusion of a vessel already narrowed by chronic peripheral atherosclerotic disease. Because these patients have slowly developed collateral circulation over time, the presentation is typically more gradual.
- Atheroembolism ("Blue Toe Syndrome"): Microemboli from a proximal atherosclerotic plaque or aneurysm shower distally, causing cool, painful, cyanotic toes despite the presence of palpable distal macroscopic pulses.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Immediate Stabilization: Assess ABCs, establish continuous cardiac monitoring, and place two large-bore IVs.
- First-Line Pharmacotherapy: Administer immediate systemic anticoagulation with Intravenous Heparin to halt the propagation of the clot.
- Surgical Imperative: Obtain an immediate, emergent consultation with vascular surgery. Patients with limb-threatening ischemia (indicated by sensory or motor deficits) require emergent operative intervention—typically a Fogarty catheter embolectomy for emboli, or thrombectomy combined with bypass grafting for in situ thrombosis.
- Medical Alternative (With Caution): Intra-arterial thrombolytic therapy can be considered for non-limb-threatening cases, but it takes 6 to 72 hours to work and is strictly contraindicated if the limb is in immediate jeopardy.
3. THE DIAGNOSTIC GRID (Differential Diagnosis & Workup)

- Compartment Syndrome: Causes the "6 Ps," though pressure and extreme pain out of proportion are the hallmarks.
- Prioritized Diagnostic Workup:
- Clinical Diagnosis: The diagnosis of acute arterial occlusion is reliably made by history and physical examination alone. You must rapidly assess the "6 Ps": Pain, Pallor, Pulselessness, Poikilothermia, Paresthesias, and Paralysis.
- Do Not Delay for Imaging: If the patient has limb-threatening ischemia, confirmatory tests like formal arteriograms or CT angiograms are unnecessary, represent a dangerous delay, and increase the limb's ischemic time.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- 12-Lead ECG: Critically evaluate the ECG for an underlying embolic source. Look for Atrial Fibrillation or evidence of an acute/recent Myocardial Infarction, which predisposes the patient to left ventricular thrombus.
- Point-of-Care Ultrasound (POCUS): Emergency physician–performed POCUS focusing on the aorta, iliac vessels, and femoral artery bifurcation can rapidly localize emboli or thromboses. Transthoracic echocardiography can be used to detect a central embolic source, such as a hypokinetic LV or an outright mural thrombus.
- Arteriogram/CTA Findings: If imaging is obtained (only in stable, non-limb-threatening cases), an embolus shows a sudden, abrupt cutoff in blood flow with minimal diseased vessel and no collaterals. In situ thrombosis shows widespread atherosclerotic disease, collaterals, and gradual narrowing.
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
Risk stratification is based on determining tissue viability through the physical examination:
- Viable (No immediate threat): Pain, pallor, and pulselessness may be present, but motor and sensory exams are completely intact.
- Jeopardized/Limb-Threatening: The presence of paresthesias (loss of light touch sensation) or paralysis strictly indicates limb-threatening ischemia. These specific deficits demand an emergent trip to the operating room for revascularization.
- Irreversible: Profound, dense paralysis with muscle rigor (rigor mortis of the limb). These patients require primary amputation due to unacceptable revascularization risks (e.g., massive reperfusion injury/rhabdomyolysis).
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- Cognitive Trap (The Delay for Imaging): Ordering an arteriogram or CT angiogram before consulting surgery in a patient with new limb paralysis or paresthesias. Correction: Any additional diagnostic evaluation beyond the clinical exam in a limb-threat scenario constitutes an unnecessary delay and risks irreversible limb loss.
- Cognitive Trap (Thrombolytics for Dead Limbs): Recommending intra-arterial thrombolytics for a patient who cannot feel or move their foot. Correction: Thrombolysis requires 6 to 72 hours to achieve reperfusion and is strictly contraindicated when immediate, limb-threatening ischemia is present.
- Critical Action: Explicitly check for light touch sensation. The loss of light touch is the cardinal harbinger of jeopardized tissue viability.
