- Point-of-Care Ultrasound (POCUS) Findings:
- Neck POCUS (for Epiglottitis): Place a linear transducer in a transverse view just below the level of the hyoid bone. An anterior-posterior (AP) epiglottis diameter greater than 3.6 mm is highly suggestive of epiglottitis. In the midsagittal view, look for the "Alphabet P sign", created by the shadow of the hyoid bone and the edematous, hypoechoic epiglottis.
- Intraoral (Transoral) POCUS (for PTA): Using an endocavitary probe, scan the tonsillar fossa. PTA is confirmed by a well-defined, hypoechoic or anechoic fluid collection with surrounding hyperemia, helping to differentiate an abscess from simple peritonsillar cellulitis and assisting in ultrasound-guided needle aspiration.
- Ludwig's Angina: Linear transducer (4-12 MHz) placed submentally can identify hypoechoic, fluid-filled collections in the submandibular and sublingual spaces, and can be used to monitor airway diameter.
- ECG Diagnostics:
- While normal in most localized throat infections, obtain an ECG to screen for myocarditis in patients presenting with suspected diphtheria (grayish-white membrane, palatal paralysis) or to rule out atypical acute coronary syndrome presenting as referred throat pain.

Clinical Probability & Management Recommendations:
- Score -1 to 1: Low probability of GABHS (\(10%\) to \(15%\)). No testing or empiric antibiotic treatment is indicated.
- Score 2: Moderate probability (\(20%\) to \(30%\)). Perform a rapid antigen detection test (RADT).
- Score 3: High probability (\(33%\)). Perform RADT.
- Score 4 or 5: Very high probability (\(40%\) to \(50%\)). Perform RADT.
- Critical Board Rule: Corticosteroids are NOT recommended as adjunctive therapy for patients with mild-to-moderate GABHS pharyngitis, and antibiotics must only be prescribed in the setting of a positive RADT or confirmatory throat culture.
2. AAP Pediatric Croup Discharge Criteria
A child with croup may be safely discharged from the ED if they meet the following strict parameters:
- No stridor at rest
- At least 3 hours since the last administration of epinephrine
- Normal color, pulse oximetry, and mental status
- Able to tolerate oral fluids
- Reliable caretaker who can recognize worsening symptoms and return to the ED
6. THE DANGER ZONE (Pitfalls & Critical Actions)
Deadly Traps and Premature Closures
- The "Normal Pharyngeal Exam" Trap: Assuming a patient with a severe sore throat and a completely normal posterior oropharynx has viral pharyngitis or is exaggerating. Critical Pitfall: Patients with early epiglottitis, retropharyngeal abscess, or Lemierre's syndrome frequently present with an entirely normal-appearing oropharynx on direct inspection.
- The CT Scan Airway Crash: Transferring an unstable patient with signs of airway compromise (drooling, stridor, tripoding) out of the ED care area to the CT or radiology suite. Any patient with a potential airway emergency must remain in a highly monitored resuscitation bay until the airway is secured.
- The Muscle Relaxant Blunder: Administering neuromuscular blocking agents (RSI) to a patient with severe trismus secondary to Ludwig's angina or a deep neck space phlegmon. Because this trismus is mechanical (due to muscle infiltration and edema), paralysis will not resolve the trismus and can precipitate a catastrophic "cannot intubate, cannot ventilate" airway crash.
- The Diagnostic Delay for Sepsis: Postponing the administration of IV fluid resuscitation or broad-spectrum antibiotics in a patient in septic shock while waiting for CT neck results.
- Dismissing Post-Tonsillectomy Bleeding: Failing to consult ENT for a minor or resolved post-tonsillectomy hemorrhage. Almost 30% of these patients will experience a second, potentially catastrophic bleeding episode within 24 hours of the sentinel bleed.

- Airway Preparation: You must document a detailed baseline airway evaluation (including the presence or absence of trismus, voice changes, tongue elevation, or neck stiffness) in every patient presenting with throat pain.
- ENT Consultation: You must obtain immediate emergency Otolaryngology (ENT) consultation for any patient presenting with Ludwig's angina, epiglottitis, retropharyngeal abscess, Lemierre's syndrome, or post-tonsillectomy hemorrhage.

"Examiner, I recognize this as a critical, life-threatening airway emergency. I am immediately placing this patient in a high-visibility resuscitation bay. I will attach continuous cardiac monitoring, pulse oximetry, and establish 2 large-bore IVs.
I will keep this patient in a strict upright position. I will obtain a difficult airway cart and prepare a cricothyrotomy kit at the bedside.
My primary survey will focus on his airway patency. I note drooling and severe trismus. Because of his mechanical trismus, I will NOT attempt a standard rapid sequence induction, as neuromuscular blockade will not relieve his mechanical obstruction and will precipitate a complete airway crash.
Instead, I am mobilizing my team for an awake fiberoptic nasotracheal intubation. I will apply high-flow oxygen, pre-oxygenate the patient, and topicalize his nasal passages using atomized lidocaine. I will administer glycopyrrolate 0.2 mg IV to dry secretions.
To optimize my intubation alignment, I will ensure the patient is positioned with his external auditory canal aligned horizontally with his sternal notch.
I am immediately consulting Otolaryngology and Anesthesia for emergency backup, bedside fiberoptic assistance, and operative intervention.
Once the airway is secured, I will order a complete blood count, BMP, blood cultures from two separate sites, and a lactate level. I will initiate empiric broad-spectrum antibiotic therapy with intravenous Ampicillin-sulbactam 3.0 g q6h.
