2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Immediate Stabilization (ABCD3EF): Secure the airway, establish bilateral IV access, and initiate continuous cardiac and pulse oximetry monitoring.
- Gastrointestinal Decontamination: If the patient presents within 1 hour of ingestion with a protected airway, administer activated charcoal. Multi-Dose Activated Charcoal (MDAC) is uniquely indicated for severe carbamazepine and phenytoin toxicity because they undergo extensive enterohepatic recirculation and have delayed gastrointestinal absorption.
- Agent-Specific Antidotal Therapy:
- Valproic Acid Toxicity: For patients with VPA-induced hyperammonemic encephalopathy or hepatotoxicity, administer Levocarnitine (L-carnitine). Dose: 100 mg/kg IV over 30 minutes, followed by 15 mg/kg every 4 hours.

- STAT Anticonvulsant Levels: Obtain quantitative serum levels of phenytoin, carbamazepine, and valproic acid.
- Ammonia Level & LFTs: Mandatory in any patient presenting with VPA ingestion or undifferentiated lethargy.
- Basic Metabolic Panel & VBG: Assess for anion gap metabolic acidosis and electrolyte derangements.
- 12-Lead ECG: Critical to assess for conduction delays.
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- 12-Lead ECG:
- For carbamazepine toxicity, scrutinize the ECG for QRS widening (>100 ms) and a prominent terminal R wave in lead aVR (>3 mm), indicative of sodium channel blockade.
- For IV phenytoin toxicity, look for profound bradycardia, AV blocks, or QT prolongation.
- Physical Examination: Look for the classic triad of phenytoin toxicity: multi-directional nystagmus, dysarthria, and severe ataxia.
- CT Head: Obtain a non-contrast head CT in any comatose patient or a patient with persistent focal deficits to rule out an acute intracranial hemorrhage or cerebral edema (especially in VPA toxicity).
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- General Disposition Guidelines: Patients with stable vital signs, resolution of post-ictal or drug-related sedation, and therapeutic anticonvulsant levels can be discharged home after consultation with toxicology. Patients with unstable vital signs, persistent ataxia, vomiting, or new laboratory abnormalities (e.g., elevated LFTs or hyperammonemia) mandate hospital admission.
- Extracorporeal Removal (Dialysis) Criteria:
- Valproic Acid: Hemodialysis is definitively indicated for serum levels > 900 mg/L, or > 1300 mg/L if the patient is in shock, or if there is any clinical evidence of cerebral edema.
- Carbamazepine: High-flux hemodialysis or charcoal hemoperfusion is indicated for refractory seizures, life-threatening dysrhythmias, or extreme serum levels.
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- The Phenytoin Administration Trap: Pitfall: Pushing IV phenytoin faster than 50 mg/min. Critical Action: Rapid infusion causes severe hypotension and bradyarrhythmias due to the propylene glycol diluent. It can also cause severe tissue necrosis ("Purple Glove Syndrome") if extravasation occurs. Always use fosphenytoin if rapid IV loading is required.
- The Normal LFT Blindspot: Pitfall: Ruling out VPA toxicity because hepatic transaminases are normal. Critical Action: VPA-induced hyperammonemic encephalopathy can occur with completely normal LFTs. You must specifically order a serum ammonia level.
