This is significantly more common than primary AI and is most frequently caused by the abrupt cessation of chronic exogenous corticosteroid therapy. Notably, mineralocorticoid production is preserved in secondary AI because aldosterone is regulated by the renin-angiotensin-aldosterone system, not ACTH.
Acute Adrenal Crisis is a true endocrine emergency triggered by acute physiologic stress (e.g., sepsis, major trauma, surgery) in a patient with AI. Without adequate cortisol, vascular receptors lose their sensitivity to endogenous and exogenous catecholamines, leading to vasodilatory shock that is utterly refractory to standard vasopressors. In primary AI, the concurrent lack of aldosterone causes unchecked renal sodium wasting and potassium retention, compounding the crisis with profound hypovolemia.
2. THE BEDSIDE ACTION PLAN (Rapid ER Management)
- Immediate Stabilization: Assess the ABCs, establish IV/IO access, and place the patient on continuous cardiopulmonary monitoring.
- Fluid & Metabolic Resuscitation: Rapidly administer isotonic crystalloid (e.g., normal saline) to aggressively treat hypovolemia and hypotension. Check a point-of-care glucose immediately and administer 0.5–1 g/kg/dose of IV dextrose if hypoglycemia is present.

- Prioritized Diagnostic Workup:
- Do not wait for laboratory results to initiate treatment. Adrenal crisis is a clinical diagnosis.
- Serum Cortisol: The best diagnostic test for primary AI. Draw a baseline level immediately upon IV placement, before giving hydrocortisone if possible, but do not delay administration.
- Electrolyte Panel (BMP): Look for the classic metabolic triad of Hyponatremia, Hypoglycemia, and Hyperkalemia. Note: Hyperkalemia is a hallmark of primary AI, not secondary.
- Other Labs: Plasma renin activity, plasma steroid levels, and thyroid function tests can assist inpatient teams with the underlying etiology.
- Infectious Workup: Obtain blood cultures, a urinalysis, and a chest radiograph to identify the underlying physiologic stressor (e.g., occult infection).
4. THE VISUAL BOARD (ECG / POCUS / Imaging)
- 12-Lead ECG: Critically evaluate for signs of severe hyperkalemia (peaked T-waves, prolonged PR interval, widened QRS complex, or sine-wave morphology) resulting from aldosterone deficiency.
- Point-of-Care Ultrasound (POCUS): Utilize the RUSH exam. In an isolated adrenal crisis, you will typically see a hyperdynamic, under-filled left ventricle with a completely collapsible ("flat") inferior vena cava (IVC) indicating profound hypovolemia.
- Physical Exam: In primary AI (Addison's), look for characteristic hyperpigmentation of the skin and mucous membranes. In secondary AI (or Cushing's patients who abruptly stopped steroids), look for a cushingoid appearance (moon facies, dorsocervical fat pad, striae).
5. THE SCORING MATRIX (Risk Stratification & Guidelines)
- Clinical Recognition Over Scoring: There is no specific ED scoring system to "rule in" an adrenal crisis. You must rely on a high index of suspicion in any critically ill patient with pressor-dependent or refractory shock, especially those with known hypopituitarism, autoimmune disease, or a history of chronic steroid use. Just 2 weeks of exogenous corticosteroid therapy is enough to suppress the HPA axis and put the patient at risk for crisis.
- Clinical Disposition: Any patient requiring stress-dose IV steroids for adrenal crisis with hemodynamic instability requires mandatory admission to an Intensive Care Unit (ICU) or step-down unit for continuous hemodynamic monitoring, serial electrolyte checks, and treatment of the precipitating illness.
6. THE DANGER ZONE (Pitfalls & Critical Actions)
- Cognitive Trap (The Vasopressor Reflex): Reflexively escalating doses of norepinephrine or epinephrine in a hypotensive patient without considering underlying endocrine failure. Correction: Adrenal crisis patients are catecholamine-depleted; vasopressors will be highly ineffective until the patient receives IV hydrocortisone.
- Cognitive Trap (Waiting for the ACTH Test): Delaying life-saving steroid administration to perform an ACTH stimulation test or waiting for serum cortisol levels to return. Correction: ACTH administration is an evaluation tool, absolutely not appropriate for therapeutic administration during an acute crisis. Give hydrocortisone immediately.
