Section 1
The 60-Second Resuscitation Card (First 5 Minutes)
Connecting an endotracheal tube to a mechanical ventilator is not the end of resuscitation—it is a high-risk cardiopulmonary transition [1,2]. Improper initial ventilator settings cause Ventilator-Induced Lung Injury (VILI), Dynamic Hyperinflation (Auto-PEEP), or Acute Right Ventricular Collapse within minutes [1,2,3]. When a ventilated patient crashes, the first action is always to disconnect the patient from the ventilator [4].

Immediate Bedside Action & Exact Settings / Dose
Crashing Vent Immediate Disconnect (“DOPES” Step 1) OR Initial PBW Calculation:
• If Crashing on Vent (Hypotension/Desaturation/High PIP): Disconnect ETT from ventilator circuit immediately for 10–15 sec and press gently on the chest [4].
• If Initial Post-RSI Setup: Measure height with tape to calculate Predicted Body Weight (PBW) [1].
Equipment / Setup Specification
PBW Formulas:
• Men: 50 + 2.3 × (Height in inches − 60)
• Women: 45.5 + 2.3 × (Height in inches − 60) [1].
Tape measure attached to every ED ventilator.
• Men: 50 + 2.3 × (Height in inches − 60)
• Women: 45.5 + 2.3 × (Height in inches − 60) [1].
Tape measure attached to every ED ventilator.
Physiological & Mechanical Objective
Disconnecting the circuit vents trapped Auto-PEEP (restoring Guyton venous return to the right atrium) and isolates machine malfunction from patient pathology [4]. Lungs scale with height, never adipose weight [1].

Immediate Bedside Action & Exact Settings / Dose
Program Phenotype-Specific Initial Ventilator Settings:
• 1. ARDS / Sepsis (Lung-Protective): Assist-Control Volume Control (AC-VC): VT 6 mL/kg PBW (range 4–8), RR 16–22, PEEP 5–10 cmH₂O (ARDSNet table), FiO₂ titrated to SpO₂ 92%–96% [1,2,5].
• 2. Obstructive (Asthma / COPD): VT 6–8 mL/kg PBW, Low RR 8–12 breaths/min, High Inspiratory Flow 80–100 L/min (square waveform, I:E = 1:4 to 1:5), PEEP 0–5 cmH₂O [4].
• 3. Severe Metabolic Acidosis (DKA / Salicylates): VT 8 mL/kg PBW, High RR 24–30 breaths/min [4].

• 3. Severe Metabolic Acidosis (DKA / Salicylates): VT 8 mL/kg PBW, High RR 24–30 breaths/min [4].
Equipment / Setup Specification
Dual-limb heated-wire ventilator circuit with inline continuous waveform ETCO₂ and closed-system inline suction catheter (14 Fr).
Physiological & Mechanical Objective
• ARDS: Prevents volutrauma/biotraumic cytokine release (6 mL/kg NNT = 11 to save a life) [1].
• Asthma: Low RR + fast inspiratory flow maximizes Expiratory Time (TE), preventing fatal Auto-PEEP [4].
• DKA: Preserves respiratory compensation [4].

Immediate Bedside Action & Exact Settings / Dose
Execute 0.5 Sec Inspiratory & Expiratory Hold Maneuvers:
• Inspiratory Hold (0.5 sec): Measure Plateau Pressure (Pplat) — keep strictly ≤ 30 cmH₂O [1,2]. Calculate Driving Pressure (ΔP = Pplat − PEEP) — keep strictly < 15 cmH₂O [2,3].
• Expiratory Hold (0.5–1.0 sec): Measure Total PEEP and subtract set PEEP to quantify Intrinsic Auto-PEEP (PEEPᵢ) [4].
• Inspiratory Hold (0.5 sec): Measure Plateau Pressure (Pplat) — keep strictly ≤ 30 cmH₂O [1,2]. Calculate Driving Pressure (ΔP = Pplat − PEEP) — keep strictly < 15 cmH₂O [2,3].
• Expiratory Hold (0.5–1.0 sec): Measure Total PEEP and subtract set PEEP to quantify Intrinsic Auto-PEEP (PEEPᵢ) [4].
Equipment / Setup Specification
Patient must be passive/sedated during hold maneuvers (spontaneous inspiratory/expiratory efforts falsify Pplat and Auto-PEEP measurements).
Physiological & Mechanical Objective
Differentiates Airway Resistance (PIP − Pplat > 5–10 cmH₂O) from Alveolar Stiffness/Compliance (Pplat > 30 cmH₂O); ΔP < 15 cmH₂O is the strongest predictor of ARDS survival [1,2,3].

Immediate Bedside Action & Exact Settings / Dose
Execute “DOPES” Algorithm if Patient Remains Unstable:
• D (Displacement): Verify square-wave ETCO₂ & 21–23 cm lip depth.
• O (Obstruction): Pass 14 Fr suction catheter completely through ETT; administer Ketamine/Propofol if biting.
• P (Pneumothorax): Perform immediate bilateral anterior thoracic POCUS; if absent lung sliding + shock, perform immediate Finger/Needle Thoracostomy [4].
• E (Equipment): Bag manually with 100% O₂ + PEEP valve.
• S (Stacked Breaths): Prolong TE, give bronchodilators + Rocuronium 1.2 mg/kg IV [4].

Equipment / Setup Specification
14 Fr suction catheter, POCUS linear/curvilinear probe, No. 10 scalpel + Kelly clamp + 28 Fr chest tube at bedside.
Physiological & Mechanical Objective
Systematically eliminates the 5 mechanical killers of an intubated patient in under 90 seconds without waiting for a portable chest radiograph [4].

Immediate Bedside Action & Exact Settings / Dose
Analgesia-First Sedation, Asynchrony Correction & Refractory ARDS Rescue: Initiate Fentanyl (50–150 mcg/hr) + Propofol (10–50 mcg/kg/min) or Ketamine (0.5–2.0 mg/kg/hr) [4]. If double-triggering persists in ARDS, increase VT to 7 mL/kg (if Pplat ≤ 30) or switch to Pressure Support / administer Cisatracurium (15 mg bolus → 37.5 mg/hr) + Prone Positioning (16 hr/day) if PaO₂/FiO₂ < 150 mmHg [2,6,7].
Equipment / Setup Specification
Check arterial blood gas 15 min after initial setup to verify pH, PaCO₂, and PaO₂/FiO₂ ratio.
Physiological & Mechanical Objective
Eliminates breath-stacking double-triggering (which doubles VT to 12 mL/kg); prone positioning recruits dorsal atelectatic lung and reduces 28-day mortality by 50% in moderate-severe ARDS [2,6].
