Immediate Bedside Action, Drug Dose & Equipment
Airway, Resuscitative Access & Surgical Alert:
• Apply high-flow O (15 L/min NRB); prepare early videolaryngoscopic RSI if cervical/perineal involvement or refractory shock is present.
• Place 2× large-bore (14G–16G) peripheral IVs or intraosseous (IO) lines; draw 2× sets of aerobic/anaerobic blood cultures, VBG/ABG, lactate, CBC, CMP, CRP, CK, and PT/PTT/fibrinogen.
• Page Acute Care Surgery / Orthopedics STAT upon clinical suspicion—do not wait for laboratory scores or CT/MRI to consult [1,2].

Exact Physiological / Mechanical Objective & Citation
Source Control Primacy: NSTI is an intravascular thrombotic and toxin-mediated surgical emergency. Liquefactive necrosis of the deep fascia obliterates the local microcirculation (Q → 0), rendering systemic antibiotics incapable of penetrating the necrotic core without physical surgical debridement [1,2].

Immediate Bedside Action, Drug Dose & Equipment
Triple-Agent Empiric Antimicrobial Blitz (Start within <15 min):
• 1. Broad-Spectrum Cell-Wall Active Agent: Piperacillin-Tazobactam 4.5 g IV over 30 min (or Meropenem 1 g–2 g IV q8h if ESBL risk/septic shock) [1,3].
• 2. MRSA Coverage: Vancomycin 25–30 mg/kg IV loading dose (actual body weight, max 3000 mg) OR Linezolid 600 mg IV q12h [1,2].
• 3. Ribosomal Toxin Suppressor (MANDATORY): Clindamycin 900 mg IV q8h [1,2].
• 1. Broad-Spectrum Cell-Wall Active Agent: Piperacillin-Tazobactam 4.5 g IV over 30 min (or Meropenem 1 g–2 g IV q8h if ESBL risk/septic shock) [1,3].

• 3. Ribosomal Toxin Suppressor (MANDATORY): Clindamycin 900 mg IV q8h [1,2].
Exact Physiological / Mechanical Objective & Citation
Overcoming the Eagle Effect: In high-inoculum, stationary-phase Streptococcus pyogenes (GAS) or Clostridium infections, bacteria downregulate Penicillin-Binding Proteins (PBPs), making β-lactams ineffective. Clindamycin binds the 50S ribosomal subunit, halting synthesis of superantigens (SpeA/B) and Clostridial α-toxin independent of bacterial growth phase [1,2].

Immediate Bedside Action, Drug Dose & Equipment
Hemodynamic Resuscitation & Early Norepinephrine:
• Infuse Balanced Crystalloid (Lactated Ringer's or Plasma-Lyte) 30 mL/kg IV bolus rapidly via pressure bag, reassessing dynamic fluid responsiveness [3].
• Start Norepinephrine 0.05–0.5 mcg/kg/min IV peripherally or centrally if MAP <65 mmHg despite initial fluid challenge [3].
• Add Vasopressin 0.03 units/min IV if Norepinephrine exceeds 0.25 mcg/kg/min [3].
• Infuse Balanced Crystalloid (Lactated Ringer's or Plasma-Lyte) 30 mL/kg IV bolus rapidly via pressure bag, reassessing dynamic fluid responsiveness [3].
• Start Norepinephrine 0.05–0.5 mcg/kg/min IV peripherally or centrally if MAP <65 mmHg despite initial fluid challenge [3].
• Add Vasopressin 0.03 units/min IV if Norepinephrine exceeds 0.25 mcg/kg/min [3].
Exact Physiological / Mechanical Objective & Citation
Restoring Mean Systemic Filling Pressure (Pₘₛf): Massive third-spacing into fascial planes plus superantigen-induced nitric oxide (NO) vasoplegia collapses venous return (VR = [Pₘₛf − Pᵣₐ] / Rᵥ). Norepinephrine provides α₁-mediated venoconstriction and arterial tone, while V₁ₐ stimulation restores vascular sensitivity in acidemic beds [3].

Immediate Bedside Action, Drug Dose & Equipment
Bedside POCUS ("STAFF" Exam) & Finger Test:
• Perform high-frequency linear (7.5–15 MHz) ultrasound over the margin of pain: evaluate for Subcutaneous thickening, Trapped air (dirty acoustic shadowing), and Fascial Fluid (>2 mm anechoic stripe along deep fascia) [4].
• Indelibly mark skin borders; if diagnosis remains equivocal in an unstable patient, perform a bedside 2-cm local anesthetic "Finger Test" down to deep fascia [1,2].
• Perform high-frequency linear (7.5–15 MHz) ultrasound over the margin of pain: evaluate for Subcutaneous thickening, Trapped air (dirty acoustic shadowing), and Fascial Fluid (>2 mm anechoic stripe along deep fascia) [4].
• Indelibly mark skin borders; if diagnosis remains equivocal in an unstable patient, perform a bedside 2-cm local anesthetic "Finger Test" down to deep fascia [1,2].
Exact Physiological / Mechanical Objective & Citation
Direct Fascial Interrogation: Cutaneous erythema lags behind deep fascial spread. POCUS detects micro-gas and perifascial fluid before radiographic changes appear [4]. Loss of normal tissue resistance on blunt finger dissection with "dishwater" purulence confirms NSTI at the bedside [1,2].

Immediate Bedside Action, Drug Dose & Equipment
Screen for Sepsis Mimics & Adjunctive Toxin Neutralization:
• Rule out NSTI/Sepsis Mimics: Check core temperature (>40.5°C in NMS/Serotonin Syndrome/Heat Stroke), review medication chart (neuroleptics, SSRIs, checkpoint inhibitors), check TSH/cortisol, and inspect lesions for Pyoderma Gangrenosum (violaceous undermined borders—worsens with debridement via pathergy) [5].
• If refractory Streptococcal Toxic Shock Syndrome (STSS), administer IVIG 1 g/kg IV on Day 1 (then 0.5 g/kg Days 2–3) and Hydrocortisone 200 mg/day IV [1,3].

Exotoxin Neutralization & Mimic Differentiation: Polyclonal IVIG provides passive neutralizing antibodies that opsonize circulating streptococcal pyrogenic exotoxins, preventing non-specific MHC-II/TCR crosslinking [1]. Identifying non-infectious mimics prevents catastrophic iatrogenic amputation or fatal delay in antidote administration [5].
